隔药灸脐疗法干预肾气虚高龄不孕女性体外受精-胚胎移植助孕结局的随机对照研究

摘　　要：目的:探究隔药灸脐疗法对肾气虚高龄不孕女性体外受精-胚胎移植(IVF-ET)助孕结局的干预作用。方法:将66例肾气虚行IVF-ET助孕的高龄不孕女性随机分为对照组和观察组,每组33例。对照组选择GnRH-a黄体期长方案促排卵,观察组在对照组促排卵方案基础上于降调日开始加用隔药灸脐治疗,每周1次,每次灸3壮,避开月经期,用至HCG日。分别于治疗前后比较两组患者肾气虚证候积分,并对两组患者治疗后的Gn用量、Gn天数、HCG日内膜情况、E_(2)水平、获卵数、受精情况以及妊娠率进行比较。结果:治疗后,两组患者在Gn用量、Gn天数、HCG日E_(2)水平、HCG日内膜厚度、A型内膜、获卵数、2PN卵裂率方面比较差异无统计学意义;经治疗后,观察组2PN受精率、优胚率、累计妊娠率显著高于对照组(P<0.01,P<0.05),肾气虚证候积分较对照组亦显著降低(P<0.05)。结论:隔药灸脐疗法可提高肾气虚高龄患者2PN受精率、优胚率和累计妊娠率,继而改善高龄肾气虚患者IVF-ET助孕结局。Objective:To investigate whether the umbilical moxibustion therapy can improve the outcome of in vitro fertilization and embryo transfer(IVF-ET)assisted pregnancy in elderly patients with renal qi deficiency.Methods:Sixty-six elderly patients with renal qi deficiency who underwent IVF-ET assisted pregnancy were randomly divided into control group and observation group,33 cases in each group.The control group selected GnRH-a luteal phase long program for ovulation induction,and the observation group began to use intermedicinal moxibustion umbilical treatment on the day of descent on the basis of the ovulation induction program of the control group,once a week,each time three zhuang moxibustion,to avoid menstruation,until the HCG day.The score of kidney qi deficiency syndrome was compared between the two groups before and after treatment,and Gn dosage,Gn days,endometrial condition of HCG day,E,level,number of eggs harvested,fertilization and pregnancy rate were compared between the two groups after treatment.Results:After treatment,there were no significant differences in Gn dosage,Gn days,E,level on HCG day,endometrial thickness,endometrial type A,egg number and 2PN cleavage rate between 2 groups.After the treatment,the 2PN fertilization rate,optimal embryo rate and cumulative pregnancy rate in the observation group were significantly higher than those in the control group(P<0.05),and the score of kidney qi deficiency syndrome was significantly lower than that in the control group(P<0.01,P<0.05).Conclusion:The umbilical moxibustion therapy can improve 2PN fertilization rate,optimal embryo rate and cumulative pregnancy rate in elderly patients with renal qi deficiency,and then improve the outcome of IVF-ET assisted pregnancy in elderly patients with renal qi deficiency.
