长针透刺联合按动推拿治疗急性腰扭伤临床研究

摘　　要：目的 观察长针透刺联合按动推拿对急性腰扭伤临床疗效。方法 将我院急性腰扭伤患者96例随机分为对照组与观察组各48例,对照组采用按动推拿治疗,观察组加用长针透刺治疗。比较两组临床疗效,治疗前后疼痛视觉模拟评分(VAS),腰椎日本骨科学会评分(JOA),Oswestry功能障碍指数评分(ODI)、腰椎活动度,血清白介素-6(IL-6)水平,骶棘肌肌电信号波幅、反转次数以及治疗期间不良反应。结果 治疗后,观察组临床总有效率高于对照组(P<0.05);两组VAS、ODI评分均低于治疗前,JOA评分高于治疗前,观察组VAS、ODI评分均低于对照组,JOA评分高于对照组(P<0.05);两组腰椎前屈、后伸、左侧屈、右侧屈活动度均高于治疗前,观察组腰椎前屈、后伸、左侧屈、右侧屈活动度高于对照组(P<0.05);两组IL-6水平均低于治疗前,观察组IL-6水平均低于对照组(P<0.05);两组骶棘肌肌电波幅和反转次数均高于治疗前,观察组骶棘肌肌电波幅和反转次数均高于对照组(P<0.05);治疗期间两组均无不良反应发生。结论 长针透刺联合按动推拿能够减轻急性腰扭伤患者腰部疼痛、炎症反应以及腰部肌肉痉挛,改善腰椎功能障碍,值得临床推广应用。Objective To observe the clinical efficacy of long-needle penetration needling combined with push massage in treatment of acute lumbar sprain.Methods Ninety-six patients with acute lumbar sprain were randomly divided into control group and observation group with 48 cases each.Control group received massage therapy,while observation group received long-needle penetration therapy.The clinical efficacy of the two groups was compared,including scores of Visual Analog Scale(VAS)before and after treatment,Japanese Orthopedic Society of Lumbar Orthopedics(JOA),Oswestry Dysfunction Index(ODI),and lumbar range of motion,serum interleukin-6(IL-6)levels,amplitude of sacrospinalis muscle electrical signals,number of reversals,and adverse reactions during treatment.Results After treatment,the total clinical effective rate of observation group was higher than that of control group(P<0.05);the VAS and ODI scores of both groups were lower than before treatment,and were lower in observation group than control group;the JOA scores in both groups were higher than before treatment,and was higher in observation group than control group(P<0.05);the range of motion of lumbar spine flexion,extension,left flexion,and right flexion in both groups was higher than before treatment,and were higher in observation group than control group(P<0.05);the average IL-6 levels in both groups were lower than before treatment,and were lower in observation group than control group(P<0.05);the amplitude and reversal frequency of the sacrospinalis muscle electrical waves in both groups were higher than before treatment,and were higher in observation group than control group( P<0.05);during the treatment period,neither group experienced any adverse reactions. Conclusion Long-needle penetration combinedwith push massage can alleviate lower back pain,inflammatory reactions,and lower back muscle spasms in patients with acutelumbar sprain,improve lumbar dysfunction,and is worthy of clinical promotion and application.
