揿针埋植联合黄连温胆汤加减治疗女性更年期不寐临床疗效观察

摘　　要：目的观察揿针埋植联合黄连温胆汤加减用于治疗女性更年期不寐的应用指导价值。方法将我院60例女性更年期不寐患者随机分为观察组与对照组各30例。对照组口服艾司唑仑治疗,观察组采用揿针埋植联合黄连温胆汤治疗。比较两组临床疗效,中医证候积分,匹兹堡睡眠质量指数(PSQI)每个条目、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分,卵泡刺激素(FSH)、雌二醇(E_(2))水平。结果治疗后,观察组总有效率高于对照组(P<0.05);两组中医证候积分均低于治疗前,观察组中医证候积分均低于对照组(P<0.05);两组PSQI每个条目(除催眠药物外)评分均低于治疗前,观察组PSQI每个条目评分(除催眠药物外)均低于对照组(P<0.05);两组SAS、SDS评分均低于治疗前,观察组SAS、SDS评分均低于对照组(P<0.05);两组FSH水平低于治疗前,E_(2)水平均低于治疗前,观察组FSH水平低于对照组,E_(2)水平高于对照组(P<0.05)。结论采用揿针埋植联合黄连温胆汤加减治疗女性更年期不寐可显著改善患者症状,缓解不良情绪,调节性激素水平。Objective To observe the guiding value of pressing needle implantation combined with Modified Huanglian Wendan Decoction in treatment of female menopausal insomnia.Methods Sixty female patients with menopausal insomnia were randomly divided into observation group and control group,with 30 cases in each group.Control group was treated with oral estazolam,while observation group was treated with pressing needle implantation combined with Huanglian Wendan Decoction.The clinical efficacy of two groups were compared,including the scores of TCM syndrome,Pittsburgh sleep quality index(PSQI)for each item,Self rating anxiety scale(SAS),Self rating depression scale(SDS),and follicle stimulating hormone(FSH)and estradiol(E_(2))levels.Results After treatment,the total effective rate of observation group was higher than that of control group(P<0.05);both groups had lower TCM syndrome scores than before treatment,while observation group had lower TCM syndrome scores than control group(P<0.05);the PSQI scores of each item(except for hypnotic drugs)in both groups were lower than before treatment,and were lower in observation group than control group(P<0.05);the SAS and SDS scores of both groups were lower than before treatment,and were lower in observation group than control group(P<0.05);the FSH levels in both groups were lower than before treatment,and were lower in observation group than control group;the E_(2) levels were both lower than before treatment,and were higher in observation group than control group(P<0.05).Conclusion The combination of pressing needle implantation and Huanglian Wendan Decoction can significantly improve symptoms,alleviate negative emotions,and regulate sexual hormone levels in women with menopausal insomnia.
