老年肺心病并发肺部感染患者的血清miR-155和miR-21表达及其临床意义

摘　　要：目的 探究血清MicroRNA-155(miR-155)和MicroRNA-21(miR-21)在老年肺心病并发肺部感染患者表达中的临床意义。方法 选择2020年3月-2022年12月浙江省台州医院收治的肺心病并发肺部感染患者46例为感染组,单纯肺心病患者54例为非感染组,采用实时荧光定量聚合酶链法检测血清miR-155和miR-21水平,采用全自动生化分析仪检测感染指标白细胞介素-6(IL-6)、C-反应蛋白(CRP),比较患者检测结果差异,并分析感染组心功能分级、肺部感染类型与检测结果的关系。结果 感染组与非感染组肺心病病程、合并糖尿病及心功能分级比较差异有统计学意义(P<0.05);感染组miR-155、miR-21相对表达量及血清IL-6、CRP水平均高于非感染组(P<0.05);感染组中,心功能Ⅳ级者miR-155、miR-21相对表达量及血清IL-6、CRP水平均高于心功能Ⅱ、Ⅲ级者(P<0.05),心功能Ⅲ级者miR-155、miR-21相对表达量及血清IL-6、CRP水平均高于心功能Ⅱ级者(P<0.05);混合感染者miR-155相对表达量高于细菌感染者(P<0.05),miR-21相对表达量及IL-6、CRP水平高于真菌感染者(P<0.05),细菌感染者CRP水平高于真菌感染者(P<0.05);感染组miR-155、miR-21相对表达量均与IL-6、CRP呈正相关关系(P<0.05)。结论 miR-155、miR-21在老年肺心病并发肺部感染患者的血清中水平升高,且与患者心功能分级、感染类型及感染指标相关。OBJECTIVE To explore the clinical significance of expressions of serum MicroRNA-155(miR-155) and MicroRNA-21(miR-21) on pulmonary infection in elderly patients with corpulmonale. METHODS A total of 46 patients with corpulmonale complicated with pulmonary infection admitted to Taizhou Hospital of Zhejiang Province from Mar 2020 to Dec 2022 were selected as the infected group and 54 patients with simple corpulmonale were enrolled as the non-infected group. Serum miR-155 and miR-21 levels were detected by real-time fluorescence quantitative polymerase chain method. Fully automatic biochemical analyzer was used to detect infection indexes such as interleukin-6(IL-6) and C-reactive protein(CRP). The differences in detection results were compared among the patients, and the correlation of cardiac function grading, pulmonary infection types with detection results was analyzed in infected group. RESULTS There were significant differences in the course of corpulmonale, diabetes mellitus and cardiac function grading between the infected group and non-infected group(P<0.05). The relative expression levels of miR-155 and miR-21 and levels of serum IL-6 and CRP in the infected group were higher than those in the non-infected group(P<0.05). In the infected group, the relative expression levels of miR-155 and miR-21 and levels of serum IL-6 and CRP levels in patients with cardiac function grade IV were higher than those in patients with cardiac function grade Ⅱ and grade Ⅲ(P<0.05);and the above indexes in patients with cardiac function grade Ⅲ were higher than those with cardiac function grade Ⅱ(P<0.05). The relative expression level of miR-155 in patients with mixed infection was higher than that in patients with bacterial infection(P<0.05), and the relative expression level of miR-21 and levels of IL-6 and CRP were higher than those in patients with fungal infection(P<0.05).
