溴夫定治疗带状疱疹有效性与安全性的Meta分析

摘　　要：目的 系统评价溴夫定治疗带状疱疹的有效性与安全性,为循证治疗带状疱疹提供可靠依据。方法 检索中国知网、维普网、万方数据知识服务平台、CBM、PubMed、Embase、Web of Science(WOS)、Cochrane Library等数据库中公开发表的溴夫定治疗带状疱疹有效性及安全性的研究,检索时限均从建库至2021年11月。治疗组以溴夫定治疗为主,对照组治疗措施不限,采用RevMan 5.3软件进行Meta分析,客观评价临床治疗效果。结果 共纳入随机对照试验(RCTs)7篇。Meta分析结果显示:治疗组临床疗效[OR=5.79,95%CI(2.23,15.05),P=0.0003]、止痛时间[MD=-2.25,95%CI(-4.35,-0.15),P=0.04]、止疱时间[MD=-0.84,95%CI(-1.49,-0.19),P=0.01]优于对照组;2组结痂时间[MD=-0.29,95%CI(-1.20,-0.62),P=0.53]、带状疱疹后神经痛发生率[RR=0.37,95%CI(0.13,1.08),P=0.07]及不良反应发生率[OR=1.10,95%CI(0.88,1.38),P=0.40]比较差异无统计学意义。结论 溴夫定治疗带状疱疹安全有效,临床疗效、止痛时间、止疱时间优于伐昔洛韦及阿昔洛韦。Objective To systematically evaluate the effectiveness and safety of brivudine in the treatment of herpes zoster,and provide a reliable basis for evidence-based treatment of herpes zoster.Methods Publicly available studies on the efficacy and safety of bromvudine in the treatment of herpes zoster were searched in CNKI,VIP,WanFang Data,CBM,PubMed,Embase,Web of Science(WOS),Cochrane Library from the establishment of the database to November 2021.The treatment group was treated with bromvudine and the control group was treated with any treatment measure,RevMan 5.3 software was used for meta-analysis to objectively evaluate the clinical treatment effect.Results A total of 7 RCTs were included.The results of Meta-analysis showed that,the clinical efficacy[OR=5.79,95%CI(2.23,15.05),P=0.0003],pain relief time[MD=-2.25,95%CI(-4.35,-0.15),P=0.04],time to blister relief[MD=-0.84,95%CI(-1.49,-0.19),P=0.01]of the treatment group were superior to the control group;The scab time[MD=-0.29,95%CI(-1.20,-0.62),P=0.53],incidence of postherpetic neuralgia[RR=0.37,95%CI(0.13,1.08),P=0.07]and the incidence of adverse reactions[OR=1.10,95%CI(0.88,1.38),P=0.40]of the two groups were compared and the differences were not statistically significance.Conclusion Brivudine is safe and effective in the treatment of herpes zoster,and its clinical efficacy,pain relief time and blister arrest time were better than valacyclovir and aciclovir.
