血清CRP、IL-6水平与慢性乙型肝炎病毒感染患者HBV-DNA病毒载量的相关性

摘　　要：目的检测慢性乙型肝炎病毒(HBV)感染患者血清C反应蛋白(CRP)、白细胞介素-6(IL-6)水平与HBV脱氧核糖核酸(HBV-DNA)病毒载量,分析血清CRP、IL-6与HBV感染患者HBV-DNA病毒载量的关系。方法本研究为前瞻性研究,纳入2019年10月至2021年10月南阳市中心医院收治的HBV感染患者80例,入院时采集患者血液标本,测定血清CRP、IL-6及HBV-DNA载量,根据HBV-DNA检测结果将患者分为阴性组、低载量组、中载量组及高载量组;比较不同HBV-DNA载量患者的实验室指标,采用Pearson相关性、Logistic回归分析检验血清CRP、IL-6与HBV感染患者HBV-DNA病毒载量的关系。结果80例HBV感染患者中,HBV-DNA阴性12例,阳性68例;68例阳性标本中,低载量31例,中载量22例,高载量15例。不同HBV-DNA病毒载量HBV感染患者血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、白蛋白、总胆红素、CRP及IL-6水平比较,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,血清CRP、IL-6与HBV感染者HBV-DNA病毒载量均呈正相关(r均>0,P均<0.05);Logistic回归分析结果显示,血清CRP、IL-6与HBV感染者HBV-DNA病毒载量有关(P<0.05);Pearson相关性分析结果显示,HBV感染患者血清CRP与IL-6呈正相关(r>0,P<0.05)。结论血清CRP、IL-6与HBV感染患者HBV-DNA病毒载量有关,临床可通过检测血清CRP、IL-6水平,监测HBV感染患者HBV-DNA载量变化。Objective To detect the levels of serum C-reactive protein(CRP),interleukin-6(IL-6)and hepatitis B virus deoxyribonucleic acid(HBV-DNA)viral load in patients with chronic hepatitis B virus(HBV)infection,and analyze the correlation of levels of serum CRP and IL-6 with HBV-DNA viral load in HBV infected patients.Methods This study was a prospective study,including 80 patients with HBV infection treated in Nanyang Central Hospital from October 2019 to October 2021.Blood samples of selected patients were collected at admission,and serum CRP,IL-6,HBV-DNA load were measured.Patients were divided into negative group,low load group,medium load group and high load group according to HBV-DNA test results.The laboratory indexes of patients with different HBV-DNA viral load were compared.And the correlation of serum CRP and IL-6 with HBV-DNA viral load in patients with HBV infection was analyzed by Pearson correlation and logistic regression analysis.Results Among the 80 patients with HBV infection,12 cases were negative,and 68 cases were positive for HBV-DNA.Among the 68 positive samples,31 cases were with low load,22 cases were with medium load,and 15 cases were with high load.There were significant differences in levels of serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),albumin,total bilirubin,CRP and IL-6 between HBV infected patients with different HBV-DNA viral loads(P<0.05).The results of Pearson correlation analysis showed that serum CRP and IL-6 were positively correlated with HBV-DNA viral load in HBV infected patients(all r>0,all P<0.05).Results of Logistic regression analysis showed that serum CRP and IL-6 were correlated with HBV-DNA viral load in HBV infected patients(P<0.05).Results of Pearson correlation analysis showed that serum CRP was positively correlated with IL-6 in HBV infected patients(r>0,P<0.05).Conclusions Serum CRP and IL-6 are related to HBV-DNA viral load in HBV infected patients.The changes of HBV-DNA load in HBV infected patients can be monitored by detecting levels of ser
