摘 要:目的分析经阴道三维超声(3D-TVS)诊断子宫宫腔病变的效能。方法抽取2020年7月至2023年6月濮阳市妇幼保健院收治的子宫宫腔病变患者63例,所有患者均行宫腔镜检查和3D-TVS诊断,以宫腔镜检查结果为"金标准",统计宫腔镜检查和3D-TVS诊断结果,并统计3D-TVS诊断符合率和诊断效能。结果宫腔镜诊断明确子宫内膜息肉31例,子宫黏膜下肌瘤16例,宫腔粘连13例,纵隔子宫3例;采用3D-TVS诊断出子宫内膜息肉34例,子宫黏膜下肌瘤15例,宫腔粘连12例,纵隔子宫2例。3D-TVS诊断子宫宫腔病变符合率为92.06%(58/63),Kappa值为0.875,95%CI为0.708~1.041,与宫腔镜诊断一致性良好。3D-TVS诊断子宫内膜息肉灵敏度为100.00%(31/31),阳性预测值为91.18%(31/34);诊断子宫黏膜下肌瘤灵敏度为87.50%(14/16),阳性预测值为93.33%(14/15);诊断宫腔粘连灵敏度为84.62%(11/13),阳性预测值为91.67%(11/12);诊断纵隔子宫灵敏度为66.67%(2/3),阳性预测值为100.00%(2/2)。结论3D-TVS诊断子宫宫腔病变符合率高,与宫腔镜检查结果一致性良好,诊断灵敏度和阳性预测值理想。Objective To analyze the efficacy of transvaginal 3-dimensional ultrasound(3D-TVS)in the diagnosis of uterine cavity diseases.Methods A total of 63 patients with uterine cavity lesions treated in Puyang Maternity and Child Care Centers from July 2020 to June 2023 were selected,all of whom underwent hysteroscopy and 3D-TVS examinations.With the hysteroscopy results as the gold standard,the diagnosis results of hysteroscopy and 3D-TVS were counted.The diagnostic coincidence rate and diagnostic efficiency of 3D-TVS were counted.Results Hysteroscopy confirmed 31 cases of endometrial polyps,16 cases of submucosal myoma,13 cases of intrauterine adhesions and 3 cases of mediastinal uterus.3D-TVS confirmed 34 cases of endometrial polyps,15 cases of submucosal myoma,12 cases of intrauterine adhesions and 2 cases of mediastinal uterus.The coincidence rate of 3D-TVS in the diagnosis of uterine cavity lesions was 92.06%(58/63),and the Kappa value was 0.875,95%CI was 0.708-1.041,suggesting good consistency with hysteroscopy.The sensitivity of 3D-TVS in the diagnosis of endometrial polyps was 100.00%(31/31),and the positive predictive value was 91.18%(31/34).The sensitivity of 3D-TVS in the diagnosis of uterine submucosal myoma was 87.50%(14/16),and the positive predictive value was 93.33%(14/15).The sensitivity of 3D-TVS in the diagnosis of intrauterine adhesions was 84.62%(11/13),and the positive predictive value was 91.67%(11/12).The sensitivity of 3D-TVS in the diagnosis of mediastinal uterus was 66.67%(2/3),and the positive predictive value was 100.00%(2/2).Conclusions 3D-TVS has a high coincidence rate and good consistency with hysteroscopy in the diagnosis of uterine cavity lesions,with good diagnostic sensitivity and positive predictive value.
