脉冲空气波压力治疗仪预防人工全髋关节置换术后DVT的有效性分析

摘　　要：目的:探讨人工关节置换术后下肢深静脉血栓(DVT)成因与护理干预方法。方法:用便利抽样法选取医院收治的择期行人工全髋关节置换治疗的70例患者,按随机数字表法分为研究组(35例,术前使用足底泵式脉冲空气压力波治疗仪预防DVT)和对照组(35例,术前分子量肝素钙4100IU注射预防DVT),观察两组治疗前、治疗7d后腿部基本情况、凝血功能、疼痛、心理健康;记录住院时间、住院费用及DVT发生情况。结果:治疗7d后,研究组患者小腿周径、足踝周径、皮肤温度及颜色变化率、肿胀率低于对照组(P<0.05);研究组患者血浆凝血酶原时间和部分凝血酶活化时间高于对照组(P<0.05);研究组患者VAS评分、SCL-90评分低于对照组(P<0.05);研究组患者DVT发生率低于对照组,其住院时间、住院费用低于对照组,差异均有统计学意义(P<0.05)。结论:人工全髋关节置换治疗患者采用脉冲空气波压力治疗仪干预对预防其术后患DVT风险有效,改善凝血功能,促进患者康复。Objective:To explore the causes and nursing interventions of deep vein thrombosis(DVT)in lower limbs after artificial hip replacement.Methods:A convenient sampling method was used to select 70 patients who were admitted to hospital for elective total hip replacement treatment.They were randomly divided into a study group(35 patients,using a plantar pump type pulse air pressure wave therapy device to prevent DVT before surgery)and a control group(35 patients,using molecular weight heparin calcium 4100 IU injection to prevent DVT before surgery),and the basic condition of the legs,blood coagulation function,pain mental health;Record the hospitalization time,hospitalization expenses,and DVT occurrence.Results:The leg circumference,ankle circumference,skin temperature,color change rate,and swelling rate in the study group were lower than those in the control group(P<0.05);After 7 days of treatment,the plasma prothrombin time and partial thrombin activation time in the study group were higher than those in the control group(P<0.05);The VAS score and SCL-90 score in the study group were lower than those in the control group(P<0.05);The incidence of DVT in the study group was lower than that in the control group.There was a statistically significant difference between the study group and the control group in terms of hospital stay,hospital expenses,and the difference(P<0.05).Conclusion:The use of pulse air wave pressure therapy in patients undergoing total hip replacement has an impact on the risk of postoperative DVT,improves blood coagulation,and promotes rehabilitation.
