摘　　要：目的探讨超声造影(CEUS)在内镜治疗小儿非复杂性阑尾炎中的应用价值。方法回顾性分析65例经内镜治疗的非复杂性阑尾炎患儿临床及超声资料。总结CEUS的图像特征及诊断价值。结果(1)60例患儿插管成功,常规二维超声和CEUS检查阑尾显示率分别为40%(24/60)、75%(45/60)。(2)CEUS表现为39例出现阑尾形态的改变,包括阑尾外形卷曲、走行迂曲,阑尾僵直或固定,内壁毛糙或不光滑,管径增宽及内腔狭窄;32例造影显示明显或局部充盈缺损,提示粪石形成或粪渣沉积;所有患儿均未出现造影剂外漏。(3)治疗后阑尾管径、阑尾内腔及腔内充盈缺损情况较治疗前改善(P<0.01)。结论CEUS观察小儿非复杂性阑尾炎具有一定声像图特征,可用于内镜逆行阑尾炎治疗术的疗效评价。Objective To investigate the application value of contrast-enhanced ultrasound(CEUS)in the endoscopic treatment of children with uncomplicated appendicitis.Methods The clinical and ultrasonic data of 65 children with uncomplicated appendicitis were retrospectively reviewed.The CEUS image features and diagnostic value were analyzed.Results①60 patients were successfully intubated,and the the appendiceal visualization rates of 2D ultrasound and CEUS was 40%(24/60)and 75%(45/60).②In 39 cases of CEUS,the morphology of the appendix was changed,including the appendix curled,tortuosity,stiffness,rough inner wall,dilated diameter,and narrowing of the lumen.Additionally,32 cases showed obvious or local filling defects,indicating the performance of fecal stones or debris deposition.None of the children had any leakage of contrast agent.③After treatment,the size of appendix diameter,lumen and filling defects were improved compared with those before treatment(P<0.01).Conclusions CEUS has certain sonographic characteristics in the observation of pediatric uncomplicated appendicitis,which can be used to evaluate the efficacy of endoscopic retrograde appendicitis therapy.
