频谱多普勒超声对特发性羊水过多胎儿血流动力学的应用研究

摘　　要：目的 探讨多普勒超声评价特发性羊水过多孕妇胎儿血流循环变化的应用价值。方法 选择临床确诊特发性羊水过多孕妇胎儿70例(羊水过多组)和正常孕妇胎儿86例(对照组),孕周28~40周,观察两组胎儿脐动脉(UA)、大脑中动脉(MCA)、静脉导管(DV)多普勒参数及围生结局。结果 与对照组相比,特发性羊水过多组UA-PI、UA-RI显著升高(P<0.05);特发性羊水过多组MCA-PI显著降低(P<0.01);羊水过多组MCA-RI、DV-PI无统计学意义(P>0.05)。特发性羊水过多组胎儿UA-PI、UA-RI、MCA-PI三者联合预测不良结局受试者工作特征(ROC)曲线下面积为0.643(P<0.05),灵敏度为88.9%,特异度为44.8%。结论 多普勒超声可评估特发性羊水过多胎儿循环血流的变化,为早期监测宫内状况提供依据。Objective To explore the value of spectral Doppler ultrasound evaluation of fetal hemodynamics parameters change in pregnant women with idiopathic polyhydramnios.Methods Seventy singleton pregnant women diagnosed with polyhydramnios at 28-40 weeks of gestation were recruited to the study(polyhydramnios group).They were matched with 86 healthy women with normal pregnancies(control group).Using Doppler echocardiography,The Doppler indices of umbilical artery,middle cerebral artery,ductus venosus,and perinatal outcome were determined in both groups.Results The pulsatility index(PI)of middle cerebral arteries(MCA)was significantly decreased,umbilical arteries pulsatility index(UA-PI)and resistance index(UA-RI)were significantly increased in fetuses of pregnant women with idiopathic polyhydramnios compared with controls(P<0.05).The two groups had no significant differences in MCA-RI and DV-PI(P>0.05).The combination of UA-PI,UA-RI,and MCA-PI polyhydramnios group were found to be associated with adverse perinatal outcome(sensitivity:88.9%,specificity:44.8%,area under the curve:0.643,P<0.05).Conclusions Spectral Doppler ultrasound can evaluate fetal hemodynamics parameters change and provide more evidences for early intrauterine monitoring of fetus with idiopathic polyhydramnios.
