重组人表皮生长因子滴眼液联合自体血清治疗老年性白内障超声乳化术后干眼症疗效观察

摘　　要：目的:探讨老年性白内障超声乳化术后使用重组人表皮生长因子滴眼液联合自体血清方案的疗效。方法:回顾性分析110例老年白内障超声乳化术后出现干眼症的患者资料,根据患者治疗情况分为对照组(n=55)和观察组(n=55),2组患者均接受重组人表皮生长因子治疗,观察组加用自体血清。于治疗前后对比患者泪膜厚度、泪膜稳定性[基础泪液分泌试验(SIT)、泪膜破裂时间(BUT)、角膜荧光素染色评分(FL)]、泪液氧化应激指标[白介素-1β(IL-1β)、白介素-8(IL-8)、丙二醛(MAD)、脂质过氧化物(LPO)]、眼部修复指标[血管内皮生长因子(VEGF)、色素上皮衍生因子(PEDF)]水平差异,评价临床疗效,统计眼部不良反应发生情况。根据患者干眼严重程度进一步分层分析不同治疗方案在轻、中、重不同严重程度患者中的效果差异。结果:观察组泪膜厚度、SIT、BUT高于对照组(P<0.05);观察组FL低于对照组(P<0.05);观察组IL-1β、IL-8、MAD、LPO低于对照组(P<0.05);观察组VEGF低于对照组(P<0.05);观察组PEDF高于对照组(P<0.05);观察组总有效率高于对照组(P<0.05);分层分析结果显示,重组人表皮生长因子滴眼液联合自体血清对于不同严重程度患者均有良好的疗效。结论:重组人表皮生长因子滴眼液联合自体血清可改善老年性白内障超声乳化术后干眼症患者干眼症状,提高泪膜稳定性、增加泪膜厚度、抑制炎症和应激反应、修复眼部损伤。Objective:To study the therapeutic effect of recombinant human epidermal growth factor eye drops combined with autologous serum after phacoemulsification for elderly cataract.Methods:The data of 110 elderly patients with dry eye after phacoemulsification were analyzed retrospectively.The patients were divided into the control group(n=55)and the study group(n=55)according to the treatment conditions.The patients in both two groups received recombinant human epidermal growth factor treatment,and the study group added autologous serum.Differences in the tear film thickness,tear film stability[Schirmer I test(SIT),tear break-up time(BUT),corneal fluorescein staining score(FL)],and tear oxidative stress indicators[interleukin-1(IL-1β),interleukin-8(IL-8),malondialdehyde(MAD),lipid peroxide(LPO)],and eye repair indicators[vascular endothelial growth factor(VEGF),pigment epithelial derived factor(PEDF)]between the two groups of patients before and after treatment were compared to evaluate the clinical efficacy and the occurrence of adverse eye reactions was statistically analyzed.According to the severity of patients’dry eye,they were divided into mild,moderate,and severe groups and further stratified to analyze the efficacy of different plans.Results:The tear film thickness,SIT,and BUT of the study group were higher than those of the control group(P<0.05).FL in the study group was lower than that in the control group(P<0.05).The levels of IL-1β,IL-8,MAD,and LPO in the study group were lower than those in the control group(P<0.05).VEGF in the study group was lower than that in the control group(P<0.05).PEDF in the study group was higher than that in the control group(P<0.05).The total effective rate of the study group was higher than that of the control group(P<0.05).The results of the stratified analysis showed that the recombinant human epidermal growth factor eye drops combined with autologous serum had a good effect on patients with different degrees of disease.Conclusion:Recombinant human epidermal growth facto
