摘 要:目的探索并评价3D打印截骨导板在Chevron截骨术治疗足外翻畸形中的效果。方法选取2017年6月至2019年6月在贵州省骨科医院接受治疗的40例足外翻手术患者作为研究对象,随机将患者均分为两组,对照组患者行常规手术治疗,观察组患者行3D打印技术辅助截骨导板矫形治疗。对比两组患者手术时间及术中失血量;术后随访12~24个月,平均(16.4±1.05)个月,采用美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)踝-后足功能评分评价患者的足部功能,根据X线片测量外翻角(hallux valgus angle,HVA)、跖骨间角(Intermetatarsal Angle,IMA),并比较两组患者术前及末次随访的结果。结果与对照组相比,观察组的手术时间更短,手术失血量更少,差异有统计学意义(P<0.05)。术前,两组患者的HVA、IMA及AOFAS评分比较差异无统计学意义(P>0.05);术后,观察组患者的HVA、IMA均小于对照组,AOFAS评分高于对照组,差异有统计学意义(P<0.05)。结论3D打印截骨导板辅助Chevron截骨手术矫正患者足外翻畸形具有一定的可行性和有效性,是临床上促进截骨准确、优化疗效的可选方法。Objective To explore and evaluate the effect of 3D printing osteotomy guide plate in Chevron osteotomy correction of hallux valgus of foot.Methods From June 2017 to June 2019,40 patients with hallux valgus in Guizhou Orthopedic Hospital were selected and randomly divided into two groups.Patients in the control group were treated with routine surgery,while patients in the observation group were treated with 3D printing technology assisted osteotomy guide plate orthopedic treatment.The operation time and blood loss during operation were compared between the two groups.The patients were followed up for 12~24 months,with an average of(16.4±1.05)months.The foot function was evaluated by the American Orthopedic Foot and Ankle Society(AOFAS),hallux valgus angle(HVA)and intermetatarsal angle(IMA)were measured and compared according to X-rays.Results Compared with the control group,the operation time and blood loss in the observation group were shorter,and the difference between the two groups was statistically significant(P<0.05).Before the operation,the comparison of HVA,IMA and AOFAS scores between the two groups had no statistical significance(P>0.05);After the operation,the HVA and IMA of the observation group were lower than those of the control group,and the AOFAS score was higher than that of the control group,which was statistically significant(P<0.05).Conclusion 3D printing osteotomy guide plate is feasible and effective for correcting hallux valgus deformity,and it is an optional method to promote accurate osteotomy and optimize curative effect in clinic.
