彩色多普勒超声心动图对心肌梗死并发室壁瘤的诊断价值

摘　　要：目的 研究心肌梗死并发室壁瘤疾病应用心电图与彩色多普勒超声心动图诊断价值的差异。方法 选取2022年6月—2023年6月桂林医学院附属医院心内科收治的疑似心肌梗死并发室壁瘤患者61例,对所有患者均实施同样的件检查方法,以左心室造影结果为金标准。对心电图与彩色多普勒超声心动图对疾病诊断效能值进行比较。结果 左心室造影结果,61例疑似心肌梗死并发室壁瘤患者中真性室壁瘤56例,假性5例。超声心动图灵敏度98.21%、特异度100.00%、准确度98.36%,均高于心电图,差异有统计学意义(χ^(2)=14.292、4.286、18.016,P<0.05)。超声心动图在前壁及心尖、下壁、侧壁诊断符合率均高于心电图,差异有统计学意义(P<0.05)。结论 心电图与彩色多普勒超声心动图对于心肌梗死并发室壁瘤均有一定诊断价值,但后者应用优势性更强,灵敏度、特异度与准确度均高,早期诊断效果确切,利于临床尽早开展治疗,从而降低疾病病死率。Objective To study the difference in diagnostic value between the application of electrocardiography and color Doppler echocardiography in myocardial infarction complicated with ventricular wall tumor disease.Methods 61 patients with suspected myocardial infarction complicated with ventricular wall tumor admitted to the Department of Cardiology,Affiliated Hospital of Guilin Medical College,from June 2022 to June 2023 were selected,and the same piecemeal examination method was implemented in all patients,with the results of left ventriculography as the gold standard.The diagnostic efficacy values of electrocardiography and color doppler echocardiography for the diagnosis of the disease were compared.Results Left ventriculography results,56 true ventricular wall tumors and 5 false ones in 61 patients with suspected myocardial infarction complicated by ventricular wall tumor.Echocardiography had higher sensitivity of 98.21%,specificity of 100.00%,and accuracy of 98.36%than electrocardiography,and the difference was statistically significant(χ^(2)=14.292,4.286,18.016,P<0.05).Echocardiography had a higher diagnostic compliance rate than electrocardiography in the anterior and apical,inferior,and lateral walls,and the difference was statistically significant(P<0.05).Conclusion Electrocardiography and color Doppler echocardiography have certain diagnostic value for myocardial infarction complicated with ventricular wall tumor,but the latter application is more advantageous,with high sensitivity,specificity and accuracy,and the early diagnostic effect is accurate,which is conducive to the early treatment of the clinic,so as to reduce the death rate of the disease.
