左心室肥厚患者QRS波群时限与左室重构、心功能的相关性研究

摘　　要：目的 探讨与分析左心室肥厚(left ventricular hypertrophy, LVH)患者QRS波群时限与左室重构、心功能的相关性。方法 回顾性分析2022年3月—2023年3月在江苏省中医院心内科门诊及住院进行诊治的98例LVH患者,按QRS波时间分组,分为QRS波群时限>110 ms(研究组)和QRS波群时限≤110 ms(参照组),每组49例。所有LVH患者均进行心电图、超声心动图检查,观察两组的左心室舒张末期的腔内径(left ventricular internal diameter diastolic, LVIDD)、左室后壁厚度(left ventricular posterior wall thickness, LVPWD)、室间隔厚度(ventricular septal thickness, IVSD)、二尖瓣峰值速度/舒张晚期二尖瓣峰值速度(E/A)、舒张早期二尖瓣瓣口血流峰值速度/二尖瓣环舒张早期运动速度(E/e’),并进行相关性分析。结果 研究组的超声心动图指标如LVIDD、LVPWD、IVSD均显著高于参照组,E/A比值、E/e’比值显著低于参照组,差异有统计学意义(P<0.05);Pearson分析显示,QRS波群时限与IVSD呈显著正相关(P<0.05),与E/A比值、E/e’比值呈显著负相关(P<0.05)。结论 部分LVH患者伴随有心电图QRS波群时限增加,QRS波群时限延长患者出现左心室重构,LVH患者QRS波群时限与左室重构存在相关性。Objective To investigate and analyze the correlation between QRS wave group time limit and left ventricular remodeling and cardiac function in patients with left ventricular hypertrophy(LVH).Methods Ninety-eight patients with LVH who were diagnosed and treated as outpatients and inpatients in the Department of Cardiology of Jiangsu Hospital of Traditional Chinese Medicine from March 2022 to March 2023 were retrospectively analyzed and grouped according to the time of the QRS waves and divided into the QRS wave group time limit of>110 ms(study group)and QRS wave group time limit of≤110 ms(reference group),with 49 cases in each group.The left ventricular internal diameter diastolic(LVIDD),left ventricular posterior wall thickness(LVPWD),ventricular septal thickness(IVSD),mitral peak velocity/late diastolic mitral peak velocity(E/A),and early diastolic mitral orifice peak velocity/early diastolic mitral annular motion velocity(E/e')were observed in both groups,and correlation analysis was carried out.Results The echocardiographic indexes such as LVIDD,LVPWD and IVSD in the study group were significantly higher than those in the reference group,and the E/A ratio and E/e'ratio were significantly lower than those in the reference group,and the differences were statistically significant(P<0.05).Pearson analysis showed that QRS group duration was significantly positively correlated with IVSD(P<0.05),and significantly negatively correlated with E/A ratio and E/e'ratio(P<0.05).Conclusion Some patients with LVH were accompanied by an increase in the QRS wave group time limit on the electrocardiogram,and patients with prolonged QRS wave group time limit showed left ventricular remodeling,and there was a correlation between the QRS wave group time limit and left ventricular remodeling in patients with LVH.
