关节镜下Endobutton钢板固定与切开复位固定治疗后交叉韧带胫骨止点撕脱骨折的效果比较

摘　　要：目的:对比关节镜下Endobutton钢板固定与切开复位固定治疗后交叉韧带(PCL)胫骨止点撕脱骨折的效果。方法:选取2019年1月—2022年6月于常州市第一人民医院接受内固定术的PCL胫骨止点撕脱骨折患者60例为观察对象,按照随机数字表法分为对照组(切开复位固定)和观察组(关节镜下Endobutton钢板固定),各30例。比较两组术中出血量和切口长度、膝关节功能与并发症。结果:观察组术中出血量少于对照组,切口长度短于对照组,差异有统计学意义(P<0.001);术后12个月,两组Lysholm膝关节评分量表评分和关节活动度高于术前,观察组高于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:与切开复位固定相比,关节镜下Endobutton钢板固定治疗PCL胫骨止点撕脱骨折的效果确切,可以减少损伤,促进关节功能恢复,且并发症较少。Objective:To compare the effect of arthroscopic Endobutton plate fixation and incisional reduction fixation in the treatment of posterior cruciate ligament(PCL)avulsion fracture of tibial insertion.Methods:Sixty patients with PCL tibial insertion avulsion fracture who underwent internal fixation in Changzhou First People's Hospital from January 2019 to June 2022 were selected as observation subjects.They were divided into the control group(incisional reduction fixation)and the observation group(arthroscopic Endobutton plate fixation)according to the method of randomized numerical table,each with thirty cases.Intraoperative bleeding and incision length,knee function and complications were compared between the two groups.Results:Intraoperative bleeding in the observation group was less than that in the control group,and the incision length was shorter than that in the control group,with statistically significant differences(P<0.001);twelve months after surgery,the Lysholm Knee Rating Scale score and joint mobility were higher in both groups than before surgery,and the observation group was higher than the control group,with a statistically significant difference(P<0.05);comparison of incidence of complication between the two groups,the difference was not statistically significant(P>0.05).Conclusion:Compared with incisional reduction fixation,arthroscopic Endobutton plate fixation is effective in treating PCL tibial insertion avulsion fracture,which can reduce the injury and promote the recovery of joint function with fewer complications.
