祛屑止痒方对头皮银屑病(血虚风燥型)的临床研究

摘　　要：目的观察祛屑止痒方外用洗剂治疗血虚风燥型头皮银屑病的有效性,并对比治疗前后患者情志变化。方法将50例头皮银屑病患者随机分成两组,治疗组25例,使用祛屑止痒方外洗,对照组25例,使用卡泊三醇搽剂,每隔2周对比PASI评分,每隔4周对比SAS、SDS量表评分变化,8周后判定疗效。结果治疗组总有效率为100.0%(25/25),对照组总有效率为100.0%(25/25),两组疗效相当(P>0.05),但治疗组痊愈率为52%(13/25),优于对照组40%(10/25);经4周治疗,治疗组PASI评分较对照组下降更加显著,经8周治疗两组PASI评分无显著统计学意义;治疗前总焦虑者率达44.0%,总抑郁者率达28.0%,随病情好转,最终焦虑者率为16.0%,抑郁者率为10.0%;经治疗8周后两组Th17/Treg水平与治疗前对比均明显下降,且治疗组较对照组下降更加显著(P<0.05);治疗中对照组有9例患者出现头皮红斑、干燥瘙痒等不适,治疗组2例患者出现头皮干燥不适,两者相比具有显著统计学意义(P<0.05)。结论祛屑止痒方外洗治疗头皮银屑病起效较快,不良反应率较低,与卡泊三醇搽剂相比具有一定优势。Objective To observe the effectiveness of the external use of Quxie Zhiyang Formula(祛屑止痒方)in the treat⁃ment of scalp psoriasis with blood deficiency and wind dryness and compare the emotional changes of patients before and after treatment.Methods A total of 50 patients with scalp psoriasis of blood deficiency and wind dryness were randomly divided into two groups.Twenty-five patients in the treatment group were treated with Quxie Zhiyang Formula and the other twenty-five pa⁃tients in the control group were given calcipotriol liniment.The changes of psoriasis area and severity index(PASI)score every two weeks,the changes of self–rating anxiety scale(SAS)and self–rating depression scale(SDS)scores every four weeks and the efficacy after eight weeks were compared.Results The total effective rate of treatment group was comparable to that of the control group(100%vs.100%)(P>0.05),but the cure rate in the treatment group was 52%(13/25),higher than that in the control group.PASI score of the treatment group decreased more significantly than that of the control group in the first four weeks of course,but after eight weeks of the treatment,PASI score of both two groups became similar.The total rate of anxiety was 44%and the total rate of depression was 28%before treatment.After treatment,the total rate of anxiety became 16%and the total rate of depression became 10%.After eight weeks of treatment,the levels of Th17/Treg in the two groups were significantly decreased compared with those before treatment,and the decrease in the treatment group was more significant than that in the control group(P<0.05).Pruritus,erythema and dryness occurred to nine patients in the control group while dryness occurred to two patients in the treatment group and the difference was significant(P<0.05).Conclusion The effect of Quxie Zhiyang Formula in treating scalp psoriasis is faster and the adverse reaction is less than calcipotriol liniment.
