2001—2021年二滩水电站库区血吸虫病监测及传播风险分析

摘　　要：目的掌握二滩水电站建库后盐边县血吸虫病流行的变化规律,评估该区血吸虫病传播风险并提出防治措施。方法2001—2021年在雅砻江二滩水电站库区盐边县血吸虫病流行区连续监测钉螺、库区人群和家畜血吸虫病情,收集血吸虫病流行相关的自然和社会因素资料等,采用SPSS 19.0软件进行统计学分析,以P<0.05为差异有统计学意义。结果2001—2021年二滩水电站库区盐边县血吸虫病疫区有螺面积100~25800 m^(2),解剖钉螺7001只,未发现阳性钉螺。累计调查25971人,血清阳性210人,人群血清平均阳性率为0.81%,未发现病原学阳性病人。累计调查耕牛6088头,未发现病牛。库区降雨量651.9~1867.1 mm,呈增加趋势。结论二滩水电站库区盐边县血吸虫病疫区内有一定的残存钉螺,未发现血吸虫病传染源。应加强库区钉螺监测和控制,防止钉螺扩散;加强人群、动物血吸虫病监测,防止传染源输入。Objective To understand the change of schistosomiasis prevalence in Yanbian County after the construction of Ertan Hydropower Station,and to evaluate the risk of schistosomiasis transmission in the area and propose control measures.Methods Schistosomiasis disease in snails,population and livestock in the schistosomiasis endemic area in Yanbian County,Ertan Hydropower Station and Yalong River was monitored continuously from 2001 to 2021.Natural and social factors related to schistosomiasis were collected.SPSS 19.0 was used for statistical analysis,and the differences P<0.05 was considered statistically significant.Results From 2001 to 2021,there were 100-25800 square meters of snails in the schistosomiasis epidemic area of Yanbian County,Ertan Hydropower Station.7001 snails were dissected,and no positive snails were found.A total of 25971 people were investigated,210 of them were seropositive,the average seropositive rate of the population was 0.81%,and no etiological positive patients were found.A total of 6088 farm cattle were investigated,and no diseased cattle were found.The rainfall in the reservoir area was 651.9-1867.1 mm,it showed an increasing trend.Conclusions There were some snails remaining in the schistosomiasis epidemic area of Yanbian County in Ertan Hydropower Station reservoir area,and no infection source of schistosomiasis has been found.The monitoring and control of snails in reservoir area should be strengthened to prevent the spread of snails.The sureveillance of human and animal schistosomiasis should be also strengthened,so that to prevent the import of infectious sources.
