桂枝茯苓胶囊联合化疗对妊娠滋养细胞肿瘤患者的疗效影响

摘　　要：目的 观察桂枝茯苓胶囊联合化疗治疗妊娠滋养细胞肿瘤(gestational trophoblastic neoplasia, GTN)患者的临床疗效。方法 选取2019年1月至2022年6月在华侨大学医院与福建医科大学附属第二医院收治的80例GTN患者,根据随机数字表法分为对照组(单一化疗)和观察组(桂枝茯苓胶囊联合化疗),各40例。观察两组患者临床疗效、肿瘤标志物[血清β-绒毛膜促性腺激素(β-chorionic gonadotropin, β-HCG),几丁质酶-3样蛋白-40(chitinase-3-like-1 protein, YKL-40),基质金属蛋白酶-2(Matrix metalloproteinase-2, MMP-2)]水平、中医证候积分(腹部肿块、疼痛拒按、阴道出血、面白乏力)、不良反应发生情况。结果 观察组临床总有效率(100.00%)高于对照组(90.00%),差异有统计学意义(P<0.05);治疗后,观察组血清β-HCG、YKL-40、MMP-2水平均低于对照组(P<0.05);观察组中医证候积分均低于对照组(P<0.05);观察组在骨髓抑制、恶心呕吐、肝肾功能损伤等不良反应发生率均低于对照组(P<0.05)。结论 桂枝茯苓胶囊联合化疗可有效地提高GTN患者临床疗效,同时扶正固元,促进血清β-hCG、 YKL-40与MMP-2恢复,减少不良反应。Objective To observe the curative effect of Guizhi Fuling Capsule combined with chemotherapy on patients with gestational trophoblastic neoplasia(GTN).Methods A total of 80 patients with GTN admitted to Huaqiao University Hospital and the Second Affiliated Hospital of Fujian Medical University were enrolled between January 2019 and June 2022.According to random number table method,they were divided into control group(n=40,chemotherapy)and observation group(n=40,Guizhi Fuling Capsules combined with chemotherapy).The clinical curative effect,levels of tumor markers[serumβ-chorionic gonadotropin(β-HCG),chitinase-3-like-1 protein(YKL-40),matrix metalloproteinase-2(MMP-2)],scores of TCM syndromes(abdominal mass,abdominal pain,vaginal bleeding,pale complexion and fatigue)and occurrence of adverse reactions in the two groups were observed.Results The total clinical response rate in observation group was higher than that in control group(100.00%vs 90.00%,P<0.05).After treatment,levels of serumβ-HCG,YKL-40 and MMP-2 in observation group were lower than those in control group(P<0.05),and scores of TCM syndromes were also lower than those in control group(P<0.05).The incidence rates of adverse reactions such as bone marrow suppression,nausea and vomiting,liver and kidney injury in observation group were lower than in control group(P<0.05).Conclusion Guizhi Fuling Capsule combined with chemotherapy can effectively improve clinical curative effect in GTN patients,promote healthy qi and consolidate the foundation,promote recovery of serumβ-hCG,YKL-40 and MMP-2,and reduce adverse reactions.
