摘 要:目的了解广州市职业人群猪链球菌抗体阳性水平,探讨其影响因素,为预防职业人群感染猪链球菌病提供科学依据。方法采用整群抽样的方法,收集广州市某屠宰场全体员工血样,一对一调查问卷,用酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)法检测血清中猪链球菌IgG抗体水平。不同组间的抗体阳性率差异用χ^(2)检验和Fisher’s精确检验,多因素分析采用Logistic回归分析,检验水准α=0.05。结果共收集231份血清和有效问卷,职业人群猪链球菌IgG抗体阳性率为16.02%(37/231),男女性抗体阳性率分别为15.91%(28/176)、16.39%(9/55),年龄组30~39岁的IgG阳性率最高,为33.33%(12/36),60岁以上未检测出IgG抗体。屠宰场职业人群血清抗体影响因素问卷单因素分析结果显示年龄、是否接触猪、伤口类型和消毒情况均有统计学意义(P均<0.05),多因素Logistic回归分析结果显示,年龄、消毒情况是职业人群血清猪链球菌IgG抗体的影响因素(P均<0.05),与18~29岁的参考年龄相比,其他年龄段的OR>1,增加血清IgG抗体阳性的风险;与无伤口不清洗相比,伤口清洗不消毒或伤口消毒不清洗的OR>1,不清洗或不消毒是职业人群血清猪链球菌IgG抗体阳性的危险因素。结论广州市屠宰场职业人群存在猪链球菌自然感染并产生抗体,但IgG抗体水平总体偏低,接触猪的职业人群存在暴露风险,随着年龄的增加,抗体阳性率增加,伤口及时清洗和消毒能有效预防猪链球菌感染,应加屠宰场等职业人群预防猪链球菌病的宣传。Objective To determine the seropositivity rates of Streptococcus suis antibody and the influencing factors in Guangzhou,and to provide basis for the targeted prevention and control of Streptococcus suis disease x test and Fisher's exact test were used to determine the positive rate of antibody among differev groups analysis was used,and the test leel wasα=0.05.Methods A cluster sampling method was used to collect blood samples from all the employees in a slaughterhouse in Guangzhou,and a one-to-onequestionnaire survey was conducted.The serum IgG antibody level of Streptococcus suis was detectedby Enzyme-Linked dImmunosorbent Assay(ELISA).Results A total of 231 serum samples and1 valid questionnaires were collected.The positiverateof Streptococcusssuis IgGantibody was 16.02%(37/231)intheoccupational population,antibody positive rate in male and female was 15.91%(28/176)in males and 16.39%(9/55)in females,respectively,and the positive rate of IgG in 30-39 years old group was the highest,it was 33.33%(12/36).IgG antibodies were not detected in those over 60 years of age.Theresultsofunivariate analysisof thequestionnaireonantibody influencing factors in slaughterhouse occupational population showed that age,contactwith pigs,wound typeand disinfection were statistically significant(all P<0.05).And the results of multivariate logistic regression analysis showed that age and skin disinfection were the influencing factors of serum IgG antibody of Streptococcus suis in the occupational population(all P<0.05).Compared with the reference age of 18-29 years old,in other age groups,OR>1 increased the risk of serum IgG antibody positive.Compared with no wound,no cleaning or no disinfection,OR>1 were risk factorsfor positiveserum Streptococcus suis IgG antibody in the slaughterhouseeoccupational population.Conclusions Natural infectioonf Streptococcus suis and the production of antibodies against Streptococcus suis are existing in occupational population in Guangzhou,but the level of IgG antibody is generally low.There is a ex
