改良皮内缝合对颌面部创伤患者创口愈合效果及瘢痕评分的影响

摘　　要：目的 研究改良皮内缝合对颌面部创伤患者创口愈合效果及瘢痕评分的影响。方法 选取2020年1月至2023年5月我院收治的100例颌面部创伤患者为研究对象,以随机数字表法将其分为对照组和观察组,每组50例。对照组行间断缝合,观察组行改良皮内缝合。比较两组的创口愈合效果,炎症因子[C反应蛋白(CRP)、降钙素原(PCT)]水平、视觉模拟评分量表(VAS)评分、温哥华瘢痕评价量表(VSS)评分、并发症发生情况及满意度。结果 观察组的创口愈合优良率为96.00%,高于对照组的84.00%(P<0.05)。术后,两组的CRP、PCT水平均低于术前,且观察组低于对照组(P<0.05)。术后3、7、14 d,两组的VAS评分均低于术前,且观察组低于对照组(P<0.05)。术后14、21、60 d,两组的VSS评分均低于术后7 d,且观察组低于对照组(P<0.05)。观察组的并发症总发生率为2.00%,低于对照组的14.00%(P<0.05)。观察组的满意度为94.00%,高于对照组的78.00%(P<0.05)。结论 改良皮内缝合治疗颌面部创伤患者的效果显著,可促进创口愈合,减少瘢痕产生,值得推广。Objective To study the influences of modified intradermal suture on wound healing effect and scar score in patients with maxillofacial trauma.Methods A total of 100 patients with maxillofacial trauma admitted in our hospital from January 2020 to May 2023 were selected as the research objects and divided into control group and observation group by random number table method,with 50 cases in each group.The control group received intermittent suture,and the observation group received modified intradermal suture.The wound healing effect,inflammatory factors[C-reactive protein(CRP),procalcitonin(PCT)]levels,Visual Analogue Scale(VAS)score,Vancouver Scar Scale(VSS)score,complications and satisfaction were compared between the two groups.Results The excellent and good rate of wound healing of the observation group was 96.00%,which was higher than 84.00%of the control group(P<0.05).After surgery,the levels of CRP and PCT in both groups were lower than those before operation,and those in the observation group were lower than the control group(P<0.05).At 3,7 and 14 d after surgery,the VAS score of both groups was lower than that before surgery,and that in the observation group was lower than the control group(P<0.05).At 14,21 and 60 d after surgery,the VSS score of both groups was lower than that of 7 d after surgery,and that in the observation group was lower than the control group(P<0.05).The total incidence of complications of the observation group was 2.00%,which was lower than 14.00%of the control group(P<0.05).The satisfaction of the observation group was 94.00%,which was higher than 78.00%of the control group(P<0.05).Conclusion The modified intradermal suture has a remarkable effect in the treatment of patients with maxillofacial trauma,it can promote wound healing and reduce scar production,which is worth promoting.
