摘　　要：目的探讨新生儿疼痛量表(NIPS)评分联合血清C.反应蛋白(CRP)、降钙素原(PCT)对新生儿化脓性脑膜炎(NPM)患儿病情严重程度的诊断效能。方法回顾性选择2015年1月~2021年11月灵宝市妇幼保健院就诊的NPM患儿临床资料,根据患儿的病情严重程度分为轻症组(45例)和重症组(25例),收集治疗前NIPS评分、血清CRP、PCT表达及脑脊液白细胞(WBC)计数。比较两组患儿NIPS评分、血清CRP、PCT表达及脑脊液WBC计数,绘制ROC曲线分析NIPS评分、血清CRP、PCT、脑脊液WBC及联合诊断NPM病情严重程度的诊断效能。结果重症组患儿NIPS评分、血清CRP、PCT表达、脑脊液WBC计数均高于轻症组(P<0.01)。ROC曲线显示,患儿NIPS评分、血清CRP、PCT表达、脑脊液WBC计数均对NPM病情严重程度具有评估价值(P<0.01)。NIPS评分联合血清CRP、PCT对NPM患儿病情严重程度评估诊断效能高于NIPS评分联合脑脊液WBC的诊断效能(P<0.05)。结论NIPS评分联合血清CRP、PCT对NPM患儿病情严重程度具有一定诊断价值,值得临床推广。Objective To explore the diagnostic efficacy of the Newborn Infant Pain Scale(NIPS)score combined with serum C-reactive protein(CRP)and procalcitonin(PCT)in cerebrospinal fluid in the severity of neonatal purulent meningitis(NPM).Methods Clinical data of NPM children who visited Lingbao Maternal and Child Health Hospital from Jan.2015 to Nov.2021 were selected,they divided into mild(n=45)and severe(n=25)group according to the severity of illness,and pre-treatment NIPS scores,serum CRP and PCT expressions,and WBCs in cerebrospinal fluid collected.The NIPS scores,serum CRP and PCT,and WBCs in cerebrospinal fluid were compared between two groups,receiver operating characteristic(ROC)curves drawn to analyze the diagnostic efficacy of NIPS scores,serum CRP and PCT,and WBCs in cerebrospinal fluid as well as their combination in NPM severity.Results The NIPS score,serum CRP and PCT expression levels,and WBCs in cerebrospinal fluid were higher in severe than mild group(P<0.01).The ROC showed that the NIPS score,serum CRP and PCT expression levels,and WBCs in cerebrospinal fluid had estimated value for the severity of the NPM(P<0.01).The diagnostic effect of NIPS score combined with serum CRP and PCT expression levels were higher than that of NIPS score plus WBCs in cerebrospinal fluid in the severity of the NPM(P<0.05).Conclusion NIPS score combined with serum CRP and PCT expression has a certain diagnostic value for the severity of the NPM and is worthy of clinical promotion.
