通窍鼻炎胶囊联合布地奈德对变应性鼻炎的疗效及对Th17/Treg平衡的影响

摘　　要：目的探究通窍鼻炎胶囊联合布地奈德对变应性鼻炎(AR)的治疗效果。方法抽取2020年2月至2022年2月郑州人民医院AR病人143例,采用随机数字表法分为联合组(n=49)、布地奈德组(n=48)和通窍鼻炎胶囊组(n=46)。布地奈德组接受布地奈德治疗,通窍鼻炎胶囊组接受通窍鼻炎胶囊治疗,联合组接受布地奈德加用通窍鼻炎胶囊治疗。对比三组总有效率、中医症状评分(鼻塞、鼻痒、流涕、喷嚏)、γ-干扰素(IFN-γ)、外周血辅助T细胞17(Th17)、白细胞介素-17(IL-17)、调节性T细胞(Treg)、免疫球蛋白E(IgE)、转化生长因子-β_(1)(TGF-β_(1))和血清阳离子蛋白(ECP)水平变化情况。统计三组不良反应发生率。结果治疗后联合组总有效率(93.88%比83.33%、73.91%)、IFN-γ[(86.47±4.85)mg/L比(80.63±4.28)mg/L、(75.46±4.73)mg/L]、Treg[(4.35±0.57)%比(4.08±0.53)%、(3.74±0.49)%]及TGF-β_(1)水平[(34.72±4.03)ng/L比(31.27±4.35)ng/L、(28.75±4.67)ng/L]均高于布地奈德组和通窍鼻炎胶囊组(P<0.05);治疗后,三组喷嚏、鼻塞、鼻痒、流涕症状评分、IL-17、ECP、IgE、Th17水平均降低,且联合组低于布地奈德组和通窍鼻炎胶囊组(P<0.05);比较三组不良反应发生率,差异无统计学意义(P>0.05)。结论通窍鼻炎胶囊联合布地奈德治疗AR,可调节Th17/Treg平衡,改善病人临床症状,减轻炎症反应,安全可靠。Objective To explore the effect of Tongqiao Biyan capsule combined with budesonide on allergic rhinitis.Methods A total of 143 patients with AR in Zhengzhou People's Hospital from February 2020 to February 2022 were selected and randomly divid⁃ed into study group(n=49),budesonide group(n=48)and Tongqiao Biyan capsule group(n=46).The budesonide group received budesonide treatment,theTongqiao Biyan capsule groupreceived Tongqiao Biyan capsule treatment,the study group was treated with Tongqiao Biyan capsule combined with budesonide.The total effective rate,traditional Chinesemedicinesymptom score(sternutation,na⁃sal obstruction,rhinocnesmus,running nose),γ-interferon(IFN-γ),the levels of Th17 cells,interleukin-17(IL-17),Treg cells,immune globulin E(IgE),transforming growth factor-β_(1)(TGF-β_(1))and eosinophilic cationic protein(ECP)of the three groups were compared.The incidence of adverse reactions in the three groups was analyzed.Results The total effective rate(93.88%vs.83.33%,73.91%),the level of IFN-γ[(86.47±4.85)mg/L vs.(80.63±4.28)mg/L,(75.46±4.73)mg/L],Treg cells[(4.35±0.57)%vs.(4.08±0.53)%,(3.74±0.49)%]and TGF-β_(1)[(34.72±4.03)ng/L vs.(31.27±4.35)ng/L,(28.75±4.67)ng/L]of the study group were higher than those of the other two group after treatment(P<0.05).On the other hand,the scores of sternutation,nasal obstruction,rhinocnesmus and running nose,the levels of IL-17,ECP,IgE and Th17 of the study group were lower than those of the other two group after treatment(P<0.05).There was no significant difference in the incidence of adverse reactions between the three groups(P>0.05).Conclusions Tongqiao Biyan capsule combined with budesonide in the treatment of AR can effectively regulate the Th17/Treg balance and improve clinical symptoms.It can also re⁃duce inflammation with high safety.
