摘 要:目的 探究股神经阻滞联合浸润麻醉对膝关节置换术患者认知及股四头肌肌力的影响。方法 选取2022年1—11月济阳区人民医院期间收治的94例膝关节置换术患者为研究对象,电脑随机分配入组分为A组、B组,各47例,分别应用股神经阻滞麻醉、股神经阻滞麻醉联合浸润麻醉,比较两组术后疼痛程度、认知水平、股四头肌肌力及并发症发生率。结果 术后12、24、48 h时,B组静息痛、活动痛评分水平均较A组低,简易精神状况量表(Mini Mental Status Examination,MMSE)评分水平均较A组高,股四头肌肌力水平较A组高,差异有统计学意义(P<0.05);B组并发症发生率为6.38%,与A组的4.26%比较,差异无统计学意义(χ2=0.000,P>0.05)。结论 对膝关节置换术患者麻醉方案中,应用股神经阻滞联合浸润麻醉,可降低其术后疼痛程度,改善其术后认知水平及股四头肌肌力水平,且安全性良好。Objective To explore the effects of femoral nerve block combined with infiltration anesthesia on cognition and quadriceps muscle strength in patients undergoing knee replacement.Methods A total of 94 patients with knee arthroplasty admitted to Jiyang People's Hospital from January to November 2022 were selected as the study objects.The patients were randomly assigned to group A and group B,47 cases in each group,and received femoral nerve block anesthesia,femoral nerve block anesthesia combined with infiltration anesthesia,respectively.The degree of postoperative pain,cognitive level,quadriceps muscle strength and complication rate were compared between the two groups.Results At 12 h,24 h and 48 h after operation,the scores of resting pain and activity pain in group B were lower than those in group A,the scores of Simple Mental State Scale(MMSE) were higher than those in group A,and the muscle strength of quadriceps muscle was higher than that in group A,the difference was statistically significant(P<0.05).The complication rate in group B was 6.38%,compared with 4.26% in group A,and the difference was not statistically significant(χ~2=0.000,P>0.05).Conclusion Femoral nerve block combined with infiltration anesthesia can reduce the degree of postoperative pain,improve the level of postoperative cognition and quadriceps muscle strength in patients with knee joint replacement,and it is safe.
