经皮椎间孔镜下髓核摘除术治疗腰椎间盘突出症临床疗效分析

摘　　要：目的 分析腰椎间盘突出症应用经皮椎间孔镜下髓核摘除术治疗的价值。方法 随机选取2020年1月—2022年12月无锡市中医医院收治的50例腰椎间盘突出症患者为研究对象,按照不同的治疗形式分为参照组和实践组,每组25例。参照组实施小切口手术,实践组实施经皮椎间孔镜下髓核摘除术,分析两组患者腰椎功能恢复情况和临床指标变化。结果 实践组手术平均时间、切口平均长度、椎间盘切除量、下床活动平均时间等指标均优于参照组,差异有统计学意义(P<0.05);手术之前,两组患者日本骨科协会评分(Japanese Orthopaedic Association Scores,JOA)量表、Oswestry残疾指数(Oswestry Disability Index,ODI)量表、视觉模拟评分量表(Visual Analogue Scale,VAS)评分比较,差异无统计学意义(P>0.05)。手术之后,参照组的JOA、ODI、VAS量表评分均优于对照组,差异有统计学意义(P<0.05);实践组并发症发生率是12.0%,低于参照组,差异有统计学意义(χ~2=3.204,P<0.05)。结论 采取经皮椎间孔镜下髓核摘除术,可以更好地改善临床指标,缩短患者治疗时间。Objective To analyze the value of application of percutaneous intervertebral foraminal scope nucleus pulposus removal for lumbar intervertebral disc herniation.Methods 50 patients with lumbar disc herniation treated in Wuxi Hospital of Traditional Chinese Medicine from January 2020 to December 2022 were randomly selected as the study objects and divided into reference group and practice group according to different treatment forms,with 25 cases in each group.The reference group underwent small incision operation,and the practice group underwent percutaneous intervertebral foraminoscopic extraction of nucleus pulposus.The lumbar function recovery and clinical index changes of the two groups were analyzed.Results The average operation time,the average incision length,the amount of discectomy and the average time of getting out of bed in the practice group were better than those in the reference group,and the differences were statistically significant(P<0.05).Before the surgery,there were no statistically significant differences in the scores of the Japanese Orthopaedic Association Scores(JOA) scale,the Oswestry Disability Index(ODI),and Visual Analogue Scale(VAS) scale between the two groups(P>0.05).After operation,JOA,ODI and VAS scales of the reference group were better than those of the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the practice group was 12.0%,which was significantly lower than that in the reference group,and the differences were statistically significant(χ~2=3.204,P<0.05).Conclusion Adopting percutaneous intervertebral foraminal scope nucleus pulposus removal can better improve the clinical indexes and shorten the treatment time of patients.
