摘 要:目的系统评价弹性髓内钉与钢板内固定治疗儿童股骨干骨折的临床疗效。方法计算机检索PubMed、Embase、中国生物医学文献服务系统、中国知网和万方数据知识服务平台等数据库,纳入关于弹性髓内钉与钢板内固定治疗儿童股骨干骨折临床疗效的研究。对文献进行筛选、资料提取和质量评价后,采用RevMan 5.4软件进行Meta分析。结果共纳入13篇文献。Meta分析结果显示,弹性髓内钉治疗股骨干骨折在减少手术时间、术中出血量、术中透视时间、术后骨折愈合时间、完全负重时间及提高术后Kolmert评分上优于钢板内固定治疗,但并发症发生率高于钢板内固定治疗(P<0.05)。结论弹性髓内钉治疗儿童股骨干骨折的整体临床疗效优于钢板内固定术,但并发症发生率较高。临床上应根据股骨干骨折患儿的具体情况进行充分的术前评估,选择合适的手术方式。Objective To systemically evaluate the clinical effect of elastic intramedullary nail and steel plate fixation for the treatment of children with femoral shaft fracture.Methods Research related to clinical efficacy of elastic intramedullary nail and steel plate fixation for the treatment of children with femoral shaft fracture was enrolled by retrieving the databases of PubMed,Embase,China Biology Medicine Disc,China National Knowledge Infrastructure,and Wanfang Data Knowledge Service Platform,etc.After screening literature,extracting data,and evaluating quality,the RevMan 5.4 software was used for Meta-analysis.Results A total of 13 literature was enrolled.The results of Meta-analysis revealed that elastic intramedullary nail for treating femoral shaft fracture exhibited superior decreases of operation duration,intraoperative bleeding volume,intraoperative fluoroscopic frequency,postoperative fracture healing time,and full load time,as well as a superior improvement of postoperative Kolmert score as compared with steel plate fixation therapy(P<0.05);however,it interpreted a higher incidence rate of complications as compared with steel plate fixation therapy(P<0.05).Conclusion Overall clinical efficacy of elastic intramedullary nail for the treatment of children with femoral shaft fracture is superior to that of steel plate fixation,but with a relatively higher incidence rate of complications.In clinical practice,adequate preoperative evaluation should be carried out according to the specific conditions of femoral shaft fracture in children,so as to select the appropriate surgical method.
