参苓白术汤结合艾灸治疗腹泻型肠易激综合征的临床分析

摘　　要：目的分析参苓白术汤加艾灸对腹泻型肠易激综合征(IBS)的疗效。方法选取2022年1月至2023年1月淮滨县中医院收治的87例腹泻型IBS患者进行研究,按照随机数字表法分成对照组(常规西医治疗)44例和观察组(参苓白术汤加艾灸治疗)43例,比较两组临床疗效;治疗前后IBS症状严重度量表(IBS-SSS)评分和血清血管活性肠肽(VIP)、P物质(SP)、神经肽Y(NPY)水平以及不良反应情况。结果观察组治疗有效率高于对照组,差异有统计学意义(P<0.05)。治疗前,两组IBS-SSS评分(腹痛程度、腹痛频率、腹胀程度、生活影响、排便满意度)比较,差异无统计学意义(P>0.05);治疗后,观察组各项IBS-SSS评分低于对照组,差异有统计学意义(P<0.05)。治疗前,两组血清VIP、SP、NPY水平比较,差异无统计学意义(P>0.05);治疗后,观察组VIP、SP水平低于对照组,NPY水平高于对照组,差异有统计学意义(P<0.05)。两组不良反应(眩晕、头痛和便秘)比较,差异无统计学意义(P>0.05)。结论参苓白术汤加艾灸治疗对腹泻型IBS疗效理想,能控制其病情,调节胃肠激素分泌,且安全性较高。Objective To analyze the efficacy of Shenling Baizhu Decoction combined with moxibustion in the treatment of diarrhea-predominant irritable bowel syndrome(IBS).Methods 87 patients with diarrhea-predominant IBS treated in Huaibin County Hospital from January 2022 to January 2023 were selected for the study.According to the random number table method,they were divided into a control group(conventional Western medicine treatment)of 44 cases and an observation group(Shenling Baizhu Decoction combined with moxibustion treatment)of 43 cases.The clinical efficacy of the two groups was compared;the scores of the IBS Symptom Severity Scale(IBS-SSS)and the levels of serum vasoactive intestinal peptide(VIP),substance P(SP),and neuropeptide Y(NPY)before and after treatment,as well as adverse reactions,were compared.Results The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the IBS-SSS scores of abdominal pain intensity,frequency of abdominal pain,degree of abdominal distension,impact on life,and satisfaction with bowel movements between the two groups(P>0.05);after treatment,the IBS-SSS scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the levels of serum VIP,SP,and NPY between the two groups(P>0.05);after treatment,the levels of VIP and SP in the observation group were lower than those in the control group,and the level of NPY was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in adverse reactions(dizziness,headache,and constipation,etc.)between the two groups(P>0.05).Conclusion The treatment of diarrhea-predominant IBS with Shenling Baizhu Decoction combined with moxibustion has an ideal effect,can control the condition,regulate the secretion of gastrointestin
