摘 要:目的探讨超声剪切波弹性成像(SWE)测量正常新生儿脑组织弹性的复测性及正常新生儿脑组织弹性的正常值。方法70例正常新生儿为研究对象,应用SWE技术对其颅脑内3个感兴趣区(ROI)(左、右侧脑室旁脑实质和基底节区脑实质)进行弹性测量。比较不同ROI的SWE值及不同SWE值的可重复性,分析颅内脑组织的SWE杨氏模量。结果正常新生儿右侧侧脑室旁脑实质剪切波速度8.86(8.41,9.21)kPa,正常新生儿左侧侧脑室旁脑实质剪切波速度(8.11±1.37)kPa。正常新生儿左右侧侧脑室旁脑实质的剪切波速度比较差异无统计学意义(P>0.05)。正常新生儿侧脑室旁脑实质剪切波速度参考值为(8.43±1.22)kPa。左、右侧侧脑室旁脑实质的弹性成像的均值以及左侧侧脑室弹性成像的最大值参数可重复性较高,分别为0.66(0.30,0.84),0.77(0.51,0.89)和0.66(0.29,0.84)。3个ROI弹性成像的最小值和标准差的可重复性较低。尤其是基底节区弹性成像的4个参数可重复性都低。两侧室旁实质的平均杨氏模量相似(右侧为9.04 kPa,左侧为9.94 kPa),而基底神经节实质的平均杨氏模量在两次测量之间差异很大(第一次测量为7.73 kPa,第二次测量为6.40 kPa)。结论SWE测量正常新生儿脑实质弹性的可重复性及稳定性好,可以量化、准确的评估新生儿脑组织弹性;新生儿正常侧脑室旁脑实质剪切波速度参考值为(8.43±1.22)kPa。Objective To discuss the repeatability and normal value of ultrasonic shear wave elastography(SWE)measurement of normal neonatal brain parenchyma.Methods 70 normal neonates were studied.SWE technique was used to measure the elasticity of three region of interest(ROI)in the brain(left and right paraventricular parenchyma and basal ganglia parenchyma).The SWE values of different ROI and the repeatability of different SWE values were compared,and the SWE Young's modulus of intracranial brain tissue was analyzed.Results The shear wave velocity of the right paraventricular parenchyma in normal neonates was 8.86(8.41,9.21)kPa,while the shear wave velocity of the left paraventricular parenchyma in normal neonates was(8.11±1.37)kPa.There was no statistically significant difference in shear wave velocity between the left and right paraventricular parenchyma of normal neonates(P>0.05).The reference value of shear wave velocity in the paraventricular parenchyma of normal neonates was(8.43±1.22)kPa.The reproducibility of the mean values of elastography of the left and right paraventricular parenchyma as well as the maximum values of the parameters of elastography of the left lateral ventricle were high,with values of 0.66(0.30,0.84),0.77(0.51,0.89),and 0.66(0.29,0.84),respectively.Reproducibility of the minimum and standard deviation of elastography for the 3 ROIs was low.In particular,the reproducibility of all 4 parameters of elastography in the basal ganglia region was low.The mean Young's modulus of the paraventricular parenchyma was similar on both sides(9.04 kPa on the right side and 9.94 kPa on the left side),whereas that of the basal ganglia parenchyma varied considerably between the two measurements(7.73 kPa for the first measurement and 6.40 kPa for the second measurment).Conclusion The reproducibility and stability of SWE measurement of parenchymal elasticity in normal neonates are good,which can quantitatively and accurately assess the elasticity of neonatal brain tissues;the reference value of parenchymal shea
