摘　　要：目的探究腰痛宁胶囊联合脊柱微调整脊手法治疗椎间盘源性腰痛(DLBP)的临床疗效。方法选取2018年10月至2020年11月株洲市人民医院收治的106例DLBP患者作为研究对象,按照随机数字表法分为对照组与观察组,各53例。对照组行脊柱微调整脊手法治疗,观察组在对照组基础上联合腰痛宁胶囊治疗。比较两组临床疗效及治疗前后疼痛程度[视觉模拟评分法(VAS)]、腰椎功能评分[日本骨科协会(JOA)评分]、炎症因子水平[白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)]。结果观察组治疗总有效率为96.23%,高于对照组的79.25%,差异有统计学意义(P<0.05);治疗后,两组VAS评分均低于治疗前,JOA评分均高于治疗前,且观察组VAS评分低于对照组,JOA评分高于对照组,差异有统计学意义(P<0.05);治疗后,两组IL-6、TNF-α水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论腰痛宁胶囊联合脊柱微调整脊手法治疗DLBP显著,可改善患者腰椎功能,减轻机体疼痛程度,值得临床推广应用。Objective To explore the clinical efficacy of Yaotongning capsules combined with spinal fine tuning spinal manipulation in the treatment of discogenic lower back pain(DLBP).Methods 106 DLBP patients admitted to Zhuzhou People's Hospital from October 2018 to November 2020 were selected as the study subjects,they were divided into the two groups according to the random number table method,with 53 cases in each group.The control group received spinal fine-tuning and spinal manipulation treatment,while the observation group received combined treatment with Yaotongning capsules on the basis of the control group.The clinical efficacy and pain level before and after treatment were compared between the two groups(visual analog scale[VAS]),lumbar spine function score(Japanese Orthopaedic Association[JOA]score),inflammatory factor levels(interleukin-6[IL-6]and tumor necrosis factor-α[TNF-α]).Results The total effective rate in the observation group was 96.23%,which was higher than 79.25%in the control group,and the difference was statistically significant(P<0.05);after treatment,the VAS scores of the two groups were lower than those before treatment,and the JOA scores were higher than those before treatment,and the VAS score in the observation group was lower than that in the control group,and the JOA score was higher than that in the control group,the differences were statistically significant(P<0.05);after treatment,the levels of IL-6 and TNF-αof the two groups were lower than those before treatment,and the observation group was lower than in the control group,the differences were statistically significant(P<0.05).Conclusion The combination of Yaotongning capsules combined with spinal fine-tuning spinal manipulation is effective in treating DLBP,improving lumbar function and reducing body pain,it is worthy of clinical promotion and application.
