基于《脏腑标本药式》探讨密蒙花方从肝论治NPDR

摘　　要：糖尿病视网膜病变(DR)是糖尿病常见并发症,属中医“消渴目病”范畴,病因有内外,病位在目,非增殖性糖尿病视网膜病变(NPDR)以气阴两虚、目络瘀滞兼早期阳虚为基本病机,涉及五脏,以肝为关键,肝病则体用皆损,主要表现为肝气、肝血、肝阳、肝阴不足,肝郁以及肝热。治疗分虚实寒热、标本缓急,以调肝为关键。密蒙花方是名中医高健生所创,入选北京市中医药管理局首批“十病十药”,被广泛用于NPDR的临床治疗,可有效延缓疾病进程、改善视功能。《脏腑标本药式》由易水学派张元素所著,是脏腑辨证的代表性著作之一,主要阐述各脏腑虚实、标本寒热治法及用药。故本文基于《脏腑标本药式》深入探讨密蒙花方从肝论治NPDR。Diabetic retinopathy(DR)is a common complication of diabetes,falling under the category of Xiaoke Mubing in Traditional Chinese Medicine(TCM).Its etiology can be attributed to both internal and external factors,with its manifestation primarily in the eyes.Non-proliferative diabetic retinopathy(NPDR)is characterized by the basic pathogenesis of Qi and Yin deficiency,stasis in the eye collaterals,and Yang deficiency in early-stage.It involves multiple organ systems,with a key focus on liver.Liver disease in this context affects the entire body,primarily manifesting as deficiency in liver Qi,liver blood,liver Yang,liver Yin,liver stagnation,and liver heat.Treatment aims to address deficiencies and excesses,cold and heat,and focuses on regulating liver.Mimenghua Formula,created by the renowned TCM practitioner GAO Jiansheng,was selected by the Beijing Administration of Traditional Chinese Medicine as one of the first"Ten Diseases,Ten Medications."It is widely used in the clinical treatment of NPDR and has been proven effective in slowing the progression of the disease and improving visual function.Viscera Specimens Medicine Formula was authored by ZHANG Yuansu who was from the school of Yishui,one of the representative works in the field of internal organs differentiation.It primarily elaborates on the patterns of deficiency and excess in each internal organs,methods of treating the root and branch of diseases,and Chinese medications choices.Therefore,This article delves into the exploration of the application of Mimenghua Formula in the treatment of NPDR with a focus on liver-related aspects,based on the principles outlined in Viscera Specimens Medicine Formula.
