T2DM患者左心室心肌T2值与左心功能的相关性分析

摘　　要：目的探讨左心室心肌平均T_(2)值与左心功能的相关性T_(2)-mapping成像对2型糖尿病(T_(2)DM)患者左心功能衰竭风险的评估价值。方法选取92例有典型心力衰竭症状和体征的T_(2)DM患者(研究组)和30例健康成人(对照组),按照NYHA心功能分级将研究组分为NYHAⅠ-Ⅱ级组(54例)、NYHAⅢ-Ⅳ级组(38例)两个亚组。所有患者行心脏磁共振电影成像、T_(2)-mapping成像及超声心动图检查。分析研究组左心室心肌平均T_(2)值与左心功能的相关性。结果研究组和对照组左心功能的心脏磁共振(cardiovascular MRI,CMRI)与超声心动图(ultrasonocardiography,UCG)检测结果差异无统计学意义(P>0.05)。研究组各亚组左心室心肌平均T_(2)值大于对照组,T_(2)值随NYHA心功能分级增加而升高,差异有统计学意义(P<0.01)。研究组左心室心肌平均T_(2)值与左心室舒张功能呈显著负相关,且研究组各亚组左心室舒张功能均低于对照组,差异有统计学意义(P均<0.01)。研究组左心室心肌平均T_(2)值与左心室收缩功能呈显著负相关(P均<0.01),但NYHAⅠ-Ⅱ级组与对照组左心室收缩功能差异无统计学意义(P>0.05)。结论T_(2)DM左心室心肌T_(2)值与左心功能存在密切相关性,可用于预测T_(2)DM患者左心功能衰竭的风险。Objective To explore the correlation between the mean T_(2)value of the left ventricular myocardium and the left cardiac function,and to evaluate the clinical value of T_(2)-mapping imaging in patients with type 2 diabetes mellitus(T_(2)DM).Methods 92 T_(2)DM patients with symptoms and signs of typical cardiac failure(study group)and 30 healthy adults(control group)were selected.According to the cardiac function classification of New York Heart Association(NYHA),T_(2)DM group was classified as NYHA I-Ⅱgroup(54 cases),NYHAⅢ-Ⅳgroup(38 cases).All patients underwent cardiac magnetic resonance im-aging(CMRI),T_(2)-mapping imaging,and ultrasonic cardiogram(UCG)examination.The correlation between the mean T_(2)value of the left ventricular myocardium and the left cardiac function in the T_(2)DM group was analyzed.Result There was no statistical difference between CMRI and UCG results of the left cardiac function in the study and control groups(P>0.05).The mean T_(2)value of the left ventricular myocardium in each T_(2)DM subgroup was greater than that in the control group(P<0.01),and the T_(2)value increased with the increase of NYHA cardiac function classification(P<0.01).The mean T_(2)value of left ventricular myo-cardium in T_(2)DM group was significantly negatively associated with left ventricular diastolic function(P<0.01),and the left ven-tricular diastolic function in T_(2)DM group was lower than the control group(P<0.01).The mean T_(2)value of the left ventricu-lar myocardium was significantly negatively associated with left ventricular systolic function(P<0.01),but no statistical differ-ence in left ventricular systolic function between the grade NYHA I-Ⅱgroup and control group(P>0.05).Conclusion There was a close correlation between T_(2)value of the left ventricular myocardium and left cardiac function of patients with T_(2)DM,which can be used to predict the risk of left ventricular failure in patients with T_(2)DM.
