摘  要:目的利用公共数据库分析胶质母细胞瘤(GBM)免疫微环境,寻找有效评价GBM免疫微环境的方法,以期提高GBM免疫治疗的有效性和准确判断预后。方法从癌症基因组图谱(TCGA)网站下载GBM患者的临床信息、转录组数据和基因突变数据。单样本基因富集分析评估免疫细胞浸润程度;K-means法样本聚类;“limma”包计算差异基因;主成分分析法进行免疫浸润细胞(ICI)打分;“ggpubr”和“reshape”R包分析患者肿瘤突变负荷(TMB)与ICI之间相关性;“Survival”包绘制K-M生存曲线;“clusterProfiler”R包做基因GO富集分析;使用Java GESA software(version 4.1.0)和“h.all.v7.2.symbols.gmt”作为参考基因集进行基因功能富集分析。结果GBM样本可聚类分为5种免疫亚型(A-E);免疫得分与患者生存时间相关,高、低免疫得分组患者间生存时间存在明显差异(P<0.01),ICI高打分组生存时间明显较长;TMB与ICI无相关性,但低TBM-低ICI患者生存最差;高、低ICI组的基因富集情况不同。结论GBM患者的预后与肿瘤微环境中免疫细胞浸润程度及基因表达的差异有关,可以根据ICI得分高低对GBM患者预后进行预测。Objective In order to improve the effectiveness of glioblastoma(GBM)immunotherapy and accurateprognosis,the public database was used to analyze the GBM immune microenvironment and figure outa comprehensive evaluation of the GBM tumor immune microenvironment.Methods Clinical information,transcriptome data and mutation data of GBM patients were downloaded from the Cancer Genome Atlas(TCGA).Single sample gene enrichment analysis(ssGSEA)was used to evaluate the degree of immune cell infiltration.The sample clustering was performed by K-means."limma"package was usedto calculate differential genes.Principal component analysis(PCA)was used to score immunoinfiltrated cells(ICI)."ggpubr"and"Reshaping"R-packet were analyzed on the correlation between tumor mutation burden(TMB)and immune score(ICI).A KM survival curve was drawn by"Survival"package.ClusterProfiler R package was used for gene GO enrichment analysis.Java GESA software(version 4.1.0)and"H.ALE.v7.2.Symbols.gmt"were used as reference gene sets for gene functional enrichment analysis(GSEA).Results GBM samples were classified into 5 immune subtypes(A-E).E type patients had the worst overall survival.The immune score was correlated with the survival time of patients,and there were significant differences in survival time between patients with high and low immunity(P<0.01).The survival time of ICI group was obviously longer.There was no correlation between TMB and ICI,but patients with low TMBandlowICI had the worst survival.Gene enrichment was different between high ICI group and low ICI group.Conclusion The prognosis of GBM patients is related to the difference of immune cells and gene expression in the tumor microenvironment.The prognosis of GBM patients can be predicted according to the ICI score.
