基于循证的品管圈管理在促进竹叶青蛇咬伤患者肢体肿痛恢复的应用

摘　　要：目的探讨品管圈(QCC)管理在竹叶青蛇咬伤患者肢体肿胀治疗中的应用效果。方法选择我院2021年5~10月实施品管圈管理前收治的竹叶青蛇咬伤患者72例为对照组(QCC活动前),选择2022年5~10月实施品管圈管理后收治的竹叶青蛇咬伤患者72例为观察组(QCC活动后)。QCC活动前采用常规护理措施;QCC活动后采用QCC护理管理,即成立QCC组织,对QCC管理应用前竹叶青蛇咬伤患者肢体肿痛进行现状调查,找出肢体肿痛的原因,设定目标,制定对策并实施。观察比较QCC活动前后患者的肢体肿痛恢复时间、QCC圈员的能力。结果QCC活动前患者的肢体肿痛恢复时间平均(4.85±1.26)d,QCC活动后患者的肢体肿痛恢复时间平均(3.43±1.08)d,QCC活动前后比较差异具有统计学意义(P<0.01);实施QCC活动后,圈员在参与积极性、解决问题能力、沟通协调能力、自信心的养成、团队凝聚力、QCC手法等方面均有不同程度的提高,尤其在团队凝聚力和QCC手法的运用方面明显提高。结论品管圈管理可通过改进多项措施促进竹叶青蛇咬伤患者肢体肿痛的好转,同时使护理人员的工作更加积极、主动,显著提升了圈员的沟通协调能力、凝聚力、解决问题能力和品管圈手法的应用能力。Objective To investigate the effect of Quality Control Circle(QCC)management to recovery of swelling limbs in patients with Trimeresurus stejnegeri snake bite.Methods A total of 72 patients with Trimeresurus Stejnegeri snake bite before the QCC activities from May to October 2021 were used as the control group,and the conventional medical and nursing treatment was conformed according to doctor's orders or hospital's rules.A total of 72 patients with Trimeresurus stejnegeri snake bite after the QCC activities from May to October 2022 were taken as the observation roup,and the evidence-based QCC was administrated.The recovering times of swelling limbs between two groups,the multiple capability of all members between before and after QCC administration were evaluated and compared.Results The average recovering times of swelling limbs in the control group is(4.85±1.26)d,on the contrast,the time of the observation group is(3.43±1.08)d,the difference was statistically significant(P<0.01).After the implementation of QCC activities,the circle members'participation enthusiasm,problem-solving ability,communication and coordination ability,the development of self-confidence,team cohesion,QCC techniques and other aspects have been improved to varying degrees,especially in the team cohesion and the application of QCC techniques.Conclusion Evidence-based QCC activities management can improve the limb swelling pain of patients with Trimeresurus stejnegeri snake bite by improving a number of measures,and make nursing staff work more active and proactive,which significantly improves the communication and coordination ability,cohesion,problem-solving ability and application ability of QCC.
