侧俯卧位在预防ICU坠积性肺炎发生的应用研究

摘　　要：目的分析侧俯卧位预防ICU坠积性肺炎的效果与价值。方法选取2020年4月至2022年5月在我院ICU治疗的患者60例,随机分成两组各30例,侧卧组在护理中采取侧卧体位,侧俯卧组在护理中采取侧俯卧体位。比较两组患者坠积性肺炎发生情况、心率、平均动脉压、呼吸频率、血氧饱和度(SPO_(2))、氧合指数、每日吸痰次数的差异。结果侧卧组坠积性肺炎发生率为26.67%,侧俯卧组坠积性肺炎发生率为6.67%,两组比较差异有统计学意义(P<0.05);两组患者的心率、平均动脉压、呼吸频率比较,差异无统计学意义(P>0.05);侧俯卧组的SPO_(2)和氧合指数均高于侧卧组,每日吸痰次数低于侧卧组,差异均有统计学意义(均P<0.05)。结论ICU患者在护理干预中采取侧俯卧位较常规侧卧位可更有效防控坠积性肺炎的发生,有效改善患者呼吸功能,对患者身体机能恢复具有积极作用。Objective To analyze the application effect and value of lateral prone position in the prevention of hypostatic pneumonia in the intensive care unit(ICU).Methods 60 patients in ICU were selected and randomly divided into two groups,with 30 patients in the lateral lying group and 30 patients in the lateral prone group.The patients were placed in both lateral and lateral prone positions,and their general conditions at the time of enrollment were compared and observed.The number of cases of hypostatic pneumonia,imaging manifestations,vital signs,and hemodynamic indicators were observed and compared between the two groups.Results The incidence of hypostatic pneumonia,in the lateral prone group was 26.67%,while in the lateral prone group it was 6.67%.The incidence of hypostatic pneumonia in the lateral prone group was significantly lower than that in the lateral prone group(P<0.05);There was no significant difference in heart rate,mean arterial pressure,and respiratory rate between the two groups of patients(P>0.05).The SPO_(2)and oxygenation index of the lateral prone group were higher than those of the lateral prone group,and the number of sputum suctions per day was lower than that of the lateral prone group(all P<0.05).Conclusion The use of lateral prone position in ICU patient care intervention can effectively prevent and control the occurrence of hypostatic pneumonia compared to conventional lateral prone position.At the same time,the patient's respiratory function can be more effectively improved,which has an important impact on the patient's physical function recovery.
