摘  要:为探讨粪菌移植(fecal microbiota transplantation,FMT)对顽固性便秘小鼠肠道Treg/Th17平衡及相关细胞因子的影响,将40只C57BL/6小鼠随机分为4组:空白组、模型组、标准组和FMT组,每组10只。除空白组外,其他3组采用盐酸洛哌丁胺灌胃构建顽固性便秘模型。空白组、模型组小鼠自由饮水;标准组小鼠给予乳果糖灌胃;FMT组小鼠给予FMT灌胃,共7 d。记录小鼠体质量、排便量、排便频率及粪便含水量变化,进行肠道菌群分析,H-E染色观察结肠组织病理变化,FACS检测结肠组织Treg、Th17数量,ELISA检测结肠组织TGF-β、IL-10、IL-17A及IL-22水平。结果显示,与模型组比较,标准组、FMT组小鼠体质量显著升高(P<0.05),排便量、排便频率及粪便含水量均升高(P<0.05)。标准组、FMT组均能促进小鼠疣微菌科、乳杆菌科菌繁殖。与模型组比较,FMT组小鼠结肠壁基本正常,炎症消退,黏膜基本愈合。与模型组比较,标准组、FMT组小鼠结肠组织中Treg数量增加,Th17数量减少,TGF-β、IL-10水平升高,IL-17A、IL-22水平下降(P<0.05)。该研究提示,FMT可促进顽固性便秘小鼠疣微菌科、乳杆菌科菌生长,促进结肠损伤恢复,促进肠黏膜屏障功能恢复,促进Treg/Th17平衡。To investigate the effect of fecal microbiota transplantation(FMT)on intestinal Treg/Th17 balance and related cytokines in intractable constipation mice,40 C57BL/6 mice were randomly divided into four groups with 10 mice each:blank group,model group,standard group,and FMT group.The intractable constipation model was established by intragastric administration of loperamide hydrochloride in all mice except those in the blank group.Mice in all blank and model groups drank freely.Mice in the standard group were given lactulose and mice in the FMT group were given FMT by gavaging,both for 7 d.The changes of body weight,defecation volume,defecation frequency,and fecal water content were recorded and the intestinal flora was analyzed.The pathological changes of colon tissue were observed by H-E staining.The number of Treg and Th17 in colon tissue were detected by FACS.The levels of TGF-β,IL-10,IL-17A,and IL-22 in colon tissue were detected by ELISA.The result showed that compared to those in the model group,the body weight of mice in the standard group and FMT group increased significantly(P<0.05).The defecation volume,defecation frequency,and fecal water content in both the standard group and FMT group increased(P<0.05).The conditions provided by the standard group and FMT group promoted the reproduction of Verrucaceae and Lactobacillaceae in mice.Compared to those of the model group,the colonic wall of the FMT group was basically normal,the inflammation subsided and the mucosa basically healed.In addition,the number of Treg increased and Th17 decreased,the levels of TGF-β,IL-10 increased and IL-17A,IL-22 decreased(P<0.05)compared to that of the model group.The results suggest that FMT can promote the growth of Verrucaceae and Lactobacilli,the recovery of colon injury,the barrier function of the intestinal mucosa,and the balance between Treg and Th17 in intractable constipation mice.
