摘　　要：目的观察如意金黄散酊剂干预促MRSA感染大鼠创面面积、菌落及毒力因子的影响,并探讨如意金黄散酊剂干预促MRSA感染创面抑制耐药的机制。方法将41只雌性SD大鼠随机分为生理盐水组、75%酒精组、莫匹罗星组、金黄散酊组,制备大鼠MRSA感染创面模型。造模后次日,4组创面分别以生理盐水、75%酒精、莫匹罗星软膏、金黄散酊换药治疗,每日换药1次,连续14天,分别于换药第0天、第3天、第7天、第14天观察创面面积、菌落计数情况,并提取大鼠创面肉芽组织,采用Western-blot检测毒力因子SEA、SEB、金葡菌溶菌素的蛋白表达情况。结果各组间不同时间内创面面积相比没有明显差异(P>0.05);金黄散酊组MH羊血琼脂培养平板菌落计数在不同时间均低于其他组,其中第14天金黄散酊组MH羊血琼脂培养平板菌落计数较空白组有统计学意义(P<0.05);而在MRSA显色培养基平板细菌计数方面,莫匹罗星组菌落计数在不同时间均低于其他组,差异具有统计学意义(P<0.01,P<0.05);在毒力因子检测方面,不同时期毒力因子SEA,SEB、金葡菌溶菌素蛋白表达未见明显差异(P>0.05)。结论如意金黄散酊剂对于MRSA感染大鼠创面的影响主要体现在非单一菌落中多样共生细菌的体外抑制作用,而对MRSA相关SEA、SEB等毒力因子减弱及局部创面愈合作用不甚明显,考虑MRSA感染对于皮肤伤口的影响并不起主要作用。Objective To observe the effect of Ruyi Jinhuang San Tincture on the wound area,bacterial colony and virulence fac-tor of rats infected with MRSA,and to explore the mechanism of Ruyi Jinhuang San Tincture on the inhibition of drug resistance in the wound infected with MRSA.Methods SD female rats were randomly divided into three groups:blank group,75%alcohol group,mupirocin ointment group and Jinhuangsan tincture group.The rat model of MRSA infection wound was prepared.The next day after modeling,the wounds of the four groups were treated with physiological saline,75%alcohol,mupirocin ointment and Jinhuangsan tincture respectively.The dressing was changed once a day for 14 consecutive days.The wound area and bacterial count were observed on the 0,3,7 and 14 days after dressing change.Results There was no significant difference in wound area between the groups at different time(P>0.05);The colony count of MH sheep blood agar culture plate in Jinhuangsan tincture group was lower than that in other groups at different times,and the colony count of MH sheep blood agar culture plate in Jinhuan-gsan tincture group was significantly higher than that in the blank group on the 14th day(P<0.05);In terms of MRSA chromo-genic medium plate bacterial count,the colony count of the mupirocin group was lower than that of the other groups at different times,and the difference was statistically significant(P<0.01,P<0.05).In terms of virulence factor detection,there was no significant difference in the expression of virulence factors SEA,SEB and staphylococcus aureus lysozyme protein in different peri-ods(P>0.05).Conclusion The effect of Ruyi Jinhuangsan Tincture on the wound surface of rats infected with MRSA is mainly reflected in the inhibition of multiple symbiotic bacteria in non-single colonies in vitro,while it has little effect on the weakening of MRSA-related SEA,SEB and other virulence factors and local wound healing.Considering the effect of MRSA infection on skin wound,it does not play a major role.
