薄层螺旋CT多平面重建诊断外侧型腰椎间盘突出症的价值

摘　　要：目的:探究薄层螺旋电子计算机断层扫描(Computed Tomography,CT)多平面重建(Multiplane reconstruction,MPR)对外侧型腰椎间盘突出症(Lumbar disc herniation,LDH)的诊断价值。方法:于2022年1月至2023年6月期间高安市人民医院收治的LDH患者中选取80例作为研究对象,所有患者均行薄层螺旋CT MPR检查,判断其对外侧型LDH的诊断价值。结果:80例LDH患者中检出外侧型LDH 20例,阳性率为25.00%(20/80);与手术结果相比,薄层螺旋CT MPR诊断外侧型LDH的灵敏度为69.23%,特异度为98.48%,准确率为93.67%,阳性预测值90.00%,阴性预测值94.20%,与金标准具有中度一致性(Kappa=0.746);在对外侧型LDH突出部位的诊断中,薄层螺旋CT MPR检出L3~4者5例(25.00%),L4~5者14例(70.00%),L5~S1者1例(5.00%),其对各部位和总体符合率均较高,且与手术结果在L4~5、总体诊断符合率上的差异有统计学意义(P<0.05)。结论:薄层螺旋CT MPR对外侧型LDH的诊断价值较高,临床上可据此制定手术治疗方案。Objective:To explore the diagnostic value of thin-slice spiral computed tomography(CT)multiplanar reconstruction(MPR)in the diagnosis of lateral lumbar disc herniation(LDH).Methods:A total of 80 patients with LDH admitted to Gao'an People's Hospital were enrolled as the research objects between January 2022 and June 2023.All patients underwent thin-slice spiral CT MPR to determine its diagnostic value for lateral LDH.Results:In the 80 patients with LDH,there were 20 cases(25.00%(20/80))with lateral LDH.Compared with the surgical results,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of thin-slice spiral CT MPR in the diagnosis of lateral LDH were 69.23%,98.48%,93.67%,90.00%and 94.20%,showing moderate consistency with the golden standard(Kappa=0.746).In the diagnosis of lateral LDH protrusion sites,thin-slice spiral CT MPR showed that there were 5 cases(25.00%)with protrusion site at L3-4,14 cases(70.00%)at L4-5 and 1 case(5.00%)at L5-S1.There were high detection consistency rates of different sites and overall consistency rate between MPR and the golden standard.There were significant differences in coincidence rate of L4-5 protrusion and overall consistency rate between the two methods(P<0.05).Conclusion:Thin-slice spiral CT MPR has high diagnostic value for lateral LDH.Clinically,therapeutic regimens can be formulated based on it.
