内质网应激-自噬在皮瓣缺血再灌注损伤中的作用及中药干预

摘　　要：皮瓣缺血/再灌注(ischemia/reperfusion,I/R)损伤是指组织经历缺血、重新获得血液灌注或供氧后所产生的一系列损伤,是导致皮瓣坏死的主要原因,炎症反应、氧化应激、钙超载、铁死亡、细胞凋亡等是目前皮瓣缺血再灌注损伤的主要发生机制。皮瓣发生缺血、缺氧、炎症反应、氧化应激等刺激时常常会启动内质网应激-自噬途径,进而加重皮瓣I/R损伤。中医药具有多通路、多靶点协同作用,能够通过抑制内质网应激-自噬途径,改善I/R病理改变,发挥防治I/R损伤的作用。本文介绍内质网应激-自噬在I/R中的作用,并总结中药单体、中药及中药复方干预内质网应激、自噬相关因子治疗I/R的最新研究进展,以期为I/R发病机制及药物治疗的深入研究提供新思路,为临床运用中医药提高皮瓣成活率提供可靠依据。Flap ischemia-reperfusion(I/R)injury refers to a series of damages that occur when tissue experiences ischemia and then regains blood perfusion or oxygen supply.It is a primary cause of flap necrosis.Known mechanisms of flap I/R injury include inflammatory responses,oxidative stress,calcium overload,ferroptosis,and apoptosis.When flaps undergo ischemia,hypoxia,inflammatory responses,and oxidative stress,the endoplasmic reticulum stress-autophagy pathway is often activated,exacerbating I/R injury.Traditional Chinese medicine,with its multi-pathway and multi-target synergistic effects,can inhibit the endoplasmic reticulum stress-autophagy pathway,improving I/R pathological changes and playing a role in preventing and treating I/R injury.This article discusses the role of endoplasmic reticulum stress-autophagy in I/R,and summarizes the latest research reports on the intervention of monomer of Chinese medicinal herbs,Chinese herbal medicine,and Chinese herbal compound formulas in treating I/R by affecting endoplasmic reticulum stress and autophagy-related factors.This aims to provide new insights into the pathogenesis and drug treatment of I/R,and to offer reliable evidence for the clinical application of Traditional Chinese Medicine to improve flap survival rates.
