止痛消炎软膏外敷联合喜辽妥治疗动静脉内瘘术后肢体瘀肿临床研究

摘　　要：目的:观察止痛消炎软膏外敷联合序贯使用喜辽妥治疗动静脉内瘘术后肢体瘀肿的临床疗效。方法:选取80例动静脉内瘘术后肢体瘀肿患者,按随机数字表法分为观察组和对照组各40例。观察组使用止痛消炎软膏外敷联合序贯使用喜辽妥乳膏涂抹治疗,对照组用喜辽妥乳膏涂抹治疗,每天2次。2组均联合远红外线照射治疗,每次40 min,所有治疗均连续干预6 d。比较2组治疗前后肢体消肿程度、皮肤瘀肿消散面积、瘀肿消散时间、视觉模拟评分法(VAS)评分、血清高敏C-反应蛋白(hs-CRP)水平。结果:治疗后,2组肢体肿胀程度均减轻(P<0.05),且观察组改善情况优于对照组(P<0.05)。治疗48 h后,2组皮肤瘀肿面积均减小(P<0.05),且观察组小于对照组(P<0.05)。观察组瘀肿消散时间短于对照组(P<0.05)。治疗后,2组VAS评分均降低(P<0.05),且观察组低于对照组(P<0.05)。治疗后,2组hs-CRP水平均降低(P<0.05),且观察组低于对照组(P<0.05)。结论:早期应用止痛消炎软膏外敷联合序贯使用喜辽妥乳膏涂抹可有效缓解动静脉内瘘术后的瘀肿和疼痛,改善机体微炎症状态,提高术后患者舒适满意度。Objective:To observe the clinical effect of the external application of Zhitong Xiaoyan Ointment combined with the sequential use of Hirudoid on postoperative limb stasis and edema of arteriovenous fistula.Methods:A total of 80 cases of patients with postoperative limb stasis and edema of arteriovenous fistula were selected and divided into the observation group and the control group according to the random number table method,with 40 cases in each group.The observation group was treated with the external application of Zhitong Xiaoyan Ointment combined with the sequential use of Hirudoid cream,and the control group was treated with Hirudoid cream,twice a day.Both groups were additionally treated with far-infrared radiation for 40 minutes each time,and all treatments continued for 6 days.The degree of limb edema,the area and time of disappearance of skin stasis and edema,Visual Analogue Scale(VAS)scores and the levels of serum high-sensitivity C-reactive protein(hs-CRP)were compared between the two groups before and after treatment.Results:After treatment,the degree of limb edema in the two groups were decreased(P<0.05),and the improvement in the observation group was better than that in the control group(P<0.05).After 48 hours of treatment,the area of skin stasis and edema in the two groups were decreased,and the area in the observation group was smaller than that in the control group(P<0.05).The time of disappearance of skin stasis and edema in the observation group was shorter than that in the control group(P<0.05).After treatment,VAS scores in the two groups were decreased(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05).After treatment,hs-CRP in the two groups were decreased(P<0.05),and the level in the observation group was lower than that in the control group(P<0.05).Conclusion:The early application of external application of Zhitong Xiaoyan Ointment combined with the sequential use of Hirudoid cream can effectively relieve stasis,edema,and pain in patients af
