温针灸联合活血通痹汤治疗风寒湿型肩周炎临床研究

摘　　要：目的:观察温针灸联合活血通痹汤治疗风寒湿型肩周炎的临床疗效。方法:选取96例风寒湿型肩周炎患者,按随机数字表法分为联合组及对照组各48例。对照组接受活血通痹汤治疗,联合组接受温针灸联合活血通痹汤治疗,2组均连续治疗4周。比较2组临床疗效,比较2组治疗前后肩关节功能(Constant-Murley评分)、疼痛强度(PPI)、疼痛视觉模拟评分法(VAS)评分、疼痛评定指数(PRI)、中医症状积分、健康状况量表(SF-36)评分的变化。结果:联合组临床疗效总有效率为93.75%,对照组为79.17%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组肩关节活动度、疼痛、肌力、日常生活维度Constant-Murley评分均较治疗前升高(P<0.05),联合组上述4项Constant-Murley评分均高于对照组(P<0.05)。治疗后,2组PPI评分、VAS评分、PRI评分均较治疗前下降(P<0.05),联合组上述3项评分均低于对照组(P<0.05)。治疗后,2组中医症状积分均较治疗前下降(P<0.05),SF-36评分均较治疗前升高(P<0.05);联合组中医症状积分低于对照组(P<0.05),SF-36评分高于对照组(P<0.05)。结论:温针灸联合活血通痹汤治疗风寒湿型肩周炎可提升疗效,减轻疼痛,改善肩关节功能及生活质量。Objective:To observe the clinical effect of the therapy of warming-needle moxibustion combined with Huoxue Tongbi Decoction on periarthritis of shoulder of wind-cold-damp type.Methods:A total of 96 patients with periarthritis of shoulder of wind-cold-damp type were selected,and divided into the combination group and the control group according to random number table method,with 48 cases in each group.The control group was treated with Huoxue Tongbi Decoction,and the combination group was treated with warming-needle moxibustion combined with Huoxue Tongbi Decoction.Both groups were treated for 4 weeks.The clinical effects were compared between the two groups,and the changes of shoulder joint function,present pain intensity(PPI),Visual Analogue Scale(VAS),Pain Rating Index(PRI),traditional Chines medicine(TCM)symptom scores and 36-item Short Form Health Survey(SF-36)were compared before and after treatment between the two groups.Results:The total effective rate was 93.75%in the combination group and 79.17%in the control group,the difference being significant(P<0.05).After treatment,the scores of shoulder joint motion,pain,muscle strength and dimension of daily life in the two groups were increased when compared with those before treatment(P<0.05),and the above 4 scores in the combination group were higher than those in the control group(P<0.05).After treatment,the scores of PPI,VAS and PRI in the two groups were decreased when compared with those before treatment(P<0.05),and the above three scores in the combination group were lower than those in the control group(P<0.05).After treatment,the TCM symptom scores in both groups were decreased when compared with those before treatment(P<0.05),and the SF-36 scores were increased when compared with those before treatment(P<0.05);the TCM symptom scores in the combination group were lower than those in the control group(P<0.05),and the SF-36 score was higher than that in the control group(P<0.05).Conclusion:The therapy of warming-needle moxibustion combined with Huoxue Ton
