宫腔镜子宫肌瘤电切术对子宫肌瘤的疗效及对局部微循环、卵巢储备的影响分析

摘　　要：目的:探讨宫腔镜子宫肌瘤电切术治疗子宫肌瘤的疗效。方法:回顾性分析2019年9月—2021年6月黄河科技学院附属医院妇产科收治的160例子宫肌瘤患者的临床资料,根据手术方案的不同将患者分为对照组(n=80)与研究组(n=80)。对照组采用腹腔镜子宫肌瘤切除术;研究组采用宫腔镜子宫肌瘤电切术,对比两组的手术指标,治疗前与治疗后3 d的心率(HR)、促黄体生成素(LH)、血氧饱和度(SaO_(2))、促卵泡激素(FSH)、收缩压(SBP)、睾酮(T)水平。结果:两组手术时间差异无统计学意义(t=1.227,P>0.05),研究组患者术中失血量、卧床时间、住院时间小于对照组,差异有统计学意义(t=10.122、5.416、8.632,P<0.05);治疗后研究组患者LH、FSH、SaO_(2)、T水平高于对照组,差异有统计学意义(t=11.623、4.858、7.992、9.479,P<0.05),HR、SBP水平低于对照组,差异有统计学意义(t=10.864、11.093,P<0.05)。结论:宫腔镜子宫肌瘤电切术治疗子宫肌瘤的疗效良好确切,可有效保护子宫肌瘤患者卵巢储备功能,减轻手术对患者循环功能的影响。Objective:To investigate the efficacy of hysteroscopic electrosurgery of uterine fibroids in the treatment of uterine fibroids.Methods:160 patients with uterine fibroids admitted to the hospital from September 2019 to June 2021 were selected for the study,and the patients were divided into a control group(n=80)and a study group(n=80)according to the surgical protocol.The control group was treated with laparoscopic myomectomy and the study group with hysteroscopic electrosurgery of uterine fibroids.The surgical indexes,heart rate(HR),luteinizing hormone(LH),blood oxygen saturation(SaO_(2)),follicle stimulating hormone(FSH),systolic blood pressure(SBP),and testosterone(T)levels before and 3 d after treatment were compared between the two groups.Results:The difference in operative time between the two groups was not statistically significant(t=1.227,P>0.05).Intraoperative blood loss,bed rest time,and hospital stay were less in the study group than in the control group,and the differences were statistically significant(t=10.122,5.416,8.632,P<0.05).LH,FSH,SaO_(2),and T levels were higher in the study group than in the control group after treatment,with statistically significant differences(t=11.623,4.858,7.992,9.479,P<0.05).HR and SBP levels were lower than those of the control group,with statistically significant differences(t=10.864,11.093,P<0.05).Conclusion:Hysteroscopic electrosurgery for uterine fibroids has good and definite efficacy,which can effectively protect the ovarian reserve function of patients with uterine fibroids and reduce the impact of surgery on patients’circulatory function.
