摘　　要：目的 探讨甲状腺功能指标在乳糜胸(腹)疾病中的变化情况,为更全面地了解该疾病的临床特点,辅助临床诊断提供参考。方法 收集就诊于我院淋巴外科明确诊断为乳糜胸(腹)的患者,纳入疾病组,同期招募在体检中心进行健康体检者作为正常对照。比较疾病组与正常组甲状腺功能指标变化情况,比较疾病组与正常组甲状腺自身抗体检测的阳性率,利用Logistic回归及ROC曲线分析评价甲状腺功能指标对疾病的诊断价值。结果 TSH、FT3在乳糜胸(腹)组与正常组比较中差异有统计学意义(P<0.05),Logistic回归分析结果显示TSH升高和FT3降低与乳糜胸(腹)密切相关;ROC曲线分析提示TSH、FT3、TSH联合FT3对乳糜胸(腹)和正常人群的鉴别有一定价值。结论 乳糜胸(腹)患者的甲状腺功能指标较正常受试者存在增高或降低的情况,血清甲状腺功能指标检测对乳糜胸(腹)有一定的临床价值,可作为诊断的辅助参考。Objective The aim of this study is to explore changes in the levels of thyroid function indicators in chylothorax and chyloperitoneum in order to gain a more comprehensive understanding of the clinical features of the diseases and to assist in the clinical diagnosis.Methods Patients with clear diagnosis of chylothorax and chyloperitoneum attending the Lymphatic Surgery Department at Beijing Shijitan Hospital were collected and enrolled into the disease groups,while health examination subjects from the Physical Examination Center were recruited as the normal control group.The positive rate of thyroid autoantibody test was compared between the disease group and the normal group.Logistic regression and ROC curve analysis were used to evaluate the diagnostic value of thyroid function indicators for the disease.Results TSH and FT 3 were statistically significant in the disease group compared with normal group(P<0.05).Logistic regression analysis showed that increased TSH and decreased FT 3 were strongly associated with the chylothorax and chyloperitoneum.ROC curve analysis suggested that TSH,FT 3 and TSH combined with FT 3 had a certain value to differentiate disease and normal subjects.Conclusion In chylothorax and chyloperitoneum,patients have increased or decreased thyroid function indexes compared with normal subjects,and the level of serum thyroid function indexes testing have certain clinical value for the disease diagnosis,which can be used as an auxiliary reference.
