摘 要:目的研究枸橼酸咖啡因治疗新生儿呼吸暂停的效果及对临床症状改善、近期预后的影响。方法选取2019年5月~2021年5月我院收治的108例新生儿呼吸暂停患儿为研究对象,根据随机数字表法,将其分为观察组和对照组,每组各54例。观察组患儿接受枸橼酸咖啡因治疗,对照组患儿接受氨茶碱治疗。比较两组患儿的治疗总有效率、临床症状改善时间及用药后3d内呼吸暂停发作次数;比较治疗前和治疗3d后两组患儿血气指标[动脉血二氧化碳分压(PaCO_(2))、血氧分压(PaO_(2))]及血清β-内啡肽(β-EP)水平变化;观察两组患儿治疗期间不良反应发生情况。结果观察组患儿治疗总有效率显著高于对照组,不良反应总发生率显著低于对照组,差异均有统计学意义(P<0.05);观察组患儿意识好转、原始反射好转、肌张力好转时间均短于对照组,用药后3d内呼吸暂停发作次数低于对照组,差异均有统计学意义(P<0.05);治疗3d后,观察组患儿PaCO_(2)和血清β-EP水平低于对照组,PaO_(2)水平高于对照组,差异均有统计学意义(P<0.05)。结论枸橼酸咖啡因治疗新生儿呼吸暂停的效果显著,能有效改善临床症状,降低患儿呼吸暂停发作次数,且不良反应小,用药安全性高。Objective To investigate the effect of caffeine citrate in the treatment of neonatal apnea and its impact on clinical symptom improvement and short-term prognosis.Methods A total of 108 neonates with apnea admitted to our hospital from May 2019 to May 2021 were selected as the research objects.According to the random number table method,they were divided into observation group and control group,with 54 cases in each group.The observation group received caffeine citrate treatment,while the control group received aminophylline treatment.The total effective rate of treatment,clinical symptom improvement time,and the number of apnea attacks within 3 days after medication were compared between the two groups.The changes in blood gas index[arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))]and serumβ-endorphin(β-EP)levels before and 3 days after treatment were compared between the two groups.The adverse reactions during treatment were observed in both groups.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group,and the total incidence of adverse reactions was significantly lower than that in the control group(P<0.05).The improvement time of consciousness,primitive reflex,and muscle tone in the observation group was shorter than that in the control group,and the number of apnea attacks within 3 days after medication was lower than that in the control group(P<0.05).After 3 days of treatment,PaCO_(2) and serumβ-EP levels in the observation group were lower than those in the control group,while PaO_(2) level was higher than that in the control group(P<0.05).Conclusion Caffeine citrate is effective in the treatment of neonatal apnea.It can effectively improve clinical symptoms,reduce the number of apnea attacks in children,and has few adverse reactions and high medication safety.
