子宫动脉超声参数、PLGF与子痫前期发病及子代结局的关系研究

摘　　要：目的探究子宫动脉超声参数、胎盘生长因子(PLGF)与子痫前期发病及子代结局的关系。方法回顾性选取2021年7月至2022年12月在首都医科大学附属北京朝阳医院进行治疗的52例子痫前期患者纳入研究组,并随机抽取50名建档产检的健康孕妇纳入对照组,分析并比较所有研究对象孕20~24周时的子宫动脉超声参数和孕12~20周时的PLGF水平。对研究组进行随访至患者妊娠期结束,根据子代是否发生上述不良结局将其分为结局良好组(n=36)和结局不良组(n=16)。记录其子代结局,分析孕体子宫动脉超声参数及PLGF水平与子代结局的关系。采用受试者工作特征(ROC)曲线探究双侧搏动指数(UtA PI)、阻力指数(UtA RI)、PLGF水平及联合应用对孕妇子痫前期及子代结局的预测价值。结果研究组的UtA PI、UtA RI值分别为0.88±0.12、1.26±0.24,均显著高于对照组(0.65±0.11、0.97±0.16),PLGF水平为(233.16±27.59)pg/mL,显著低于对照组[(398.22±33.48)pg/mL],差异均有统计学意义(P<0.05)。ROC曲线分析结果显示:UtA PI、UtA RI、PLGF水平预测子痫前期发病的AUC值分别为0.921、0.837、0.929。结局不良组的UtA PI、UtA RI分别为0.98±0.13、1.65±0.27,均高于结局良好组(0.78±0.11、1.06±0.22),PLGF水平为(185.52±16.28)pg/mL,显著低于结局良好组[(266.26±13.26)pg/mL],差异均有统计学意义(P<0.05)。采用ROC曲线分析UtA PI、UtA RI、PLGF水平对子代不良结局的预测价值,其AUC值分别为0.883、0.926、0.830。结论子痫前期患者的UtA PI、UtA RI显著升高,PLGF水平显著下调,其水平对子痫前期的发生及子代不良结局具有一定预测价值。Objective To explore the relationship between uterine artery ultrasound parameters,placental growth factor(PLGF)and the onset of preeclampsia and the outcome of offspring.Methods A totalt of 52 preeclampsia patients treated in Beijing Chaoyang Hospital,Capital Medical University from June 2021 to December 2022 were selected and included in the study group,and 50 healthy pregnant women who were registered in hospital were randomly selected and included in the control group.The uterine artery ultrasound parameters at 20-24 weeks of pregnancy and the PLGF levels at 12-20 weeks of pregnancy among all study subjects were analyzed and compared.The study group was followed up until the end of pregnancy of the patients,and divide them into the good outcome group(n=16)and the poor outcome group(n=36)based on whether the offspring had the aforementioned adverse outcomes.And the ultrasound parameters,PLGF level and the offspring outcome were analyzed.The predictive value of pulsitility index(UtA PI),resistance index(UtA RI),PLGF,and their combination in predicting preeclampsia and offspring outcomes in pregnant women was investigated using the receiver operating characteristic(ROC)curve.Results The UtA PI and UtA RI values of the study group were 0.88±0.12 and 1.26±0.24,respectively,which were significantly higher than those of the control group(0.65±0.11 and 0.97±0.16),the level of PLGF was(233.16±27.59)pg/mL,which was significantly lower than that of the control group[398.22±33.48)pg/mL],and the differences were statistically significant(P<0.05).ROC curves were used to analyze the predictive value of UtA PI,UtA RI,and PL GF levels and the combination of preeclampsia,and the AUC values were 0.921,0.837,0.929,respectively,which had some diagnostic value.The UtA PI and UtA RI of the poor outcome group were 0.98±0.13 and 1.65±0.27,respectively,which were higher than those of the good outcome group(0.78±0.11 and 1.06±0.22),the level of PLGF was(185.52±16.28)pg/mL,which was significantly lower than that of the good o
