基于胃主通降理论探讨胃病诊疗思路

摘　　要：由于现代人们生活方式和饮食结构的变化,造成胃病的发生率呈日益上涨趋势。目前西医主要是以质子泵抑制剂等进行治疗,但远期疗效欠佳,而中医药治疗胃病无不良反应,远期疗效好,能很大程度改善患者生活质量。刘勤主任认为胃病病位在胃,但与肝脾关系密切。外邪侵袭、饮食不节、情志失畅、体虚均可导致胃之通降功能失常,治当和胃通降。根据胃以通为顺,以降为和理论,总结胃病治疗通降五法,即以补通降、驱邪通降、行气通降、祛瘀通降、以和达通,同时强调饮食调养,临床效果显著。此文通过中医病因病机、胃主通降意义、中医治疗、饮食调护、病案举隅方面介绍刘勤主任治疗胃病经验。Due to the changes of modern people's lifestyle and diet structure,the incidence of stomach disease is increasing.At present,Western medicine mainly uses proton pump inhibitors for treatment,but the long-term effect is not good,while traditional Chinese medicine treats gastric diseases without adverse reactions,and the long-term effect is good,which can greatly improve the quality of life of patients.Director Liu Qin believes that although the disease location of stomach diseases is manifested in the stomach,but it is closely related to the liver and spleen.If Invasion of external pathogens,improper diet,emotional disorder and physical deficiency,which can lead to the malfunction of dredging and descending of stomach.Therefore,the treatment of stomach diseases is that regulating and descending stomach.Basing on the theory that stomach governing the function of dredging and descending,Director Liu summarized five kinds of treatment for regulating and descending of stomach diseases,which is to restore vital energy,eliminate stasis,eliminate evil,promote the circulation of qi and regulate stomach.At the same time,Director Liu also emphasized the importance of paying attention to diet,the clinical effect is significant.This article introduced Director Liu Qin's experience in the treatment of stomach diseases,which makes a thorough analysis of the etiology and pathogenesis of traditional Chinese medicine,the significance of stomach governing the function of dredging and descending,the treatment of traditional Chinese medicine,dietary nursing and medical case.
