黄连温胆汤联合穴位埋线治疗痰热内扰型颈源性失眠临床观察

摘　　要：目的观察黄连温胆汤加减联合穴位埋线治疗痰热内扰型颈源性失眠的临床疗效。方法选取2021年1月—2022年12月来自华漕社区卫生服务中心痰热内扰型颈源性失眠患者60例,随机分为联合组和埋线组各30例。埋线组给予穴位埋线治疗,联合组给予黄连温胆汤加减联合穴位埋线治疗。2组均治疗30 d为1个疗程,共治疗8周后,比较2组匹兹堡睡眠质量指数(PSQI)评分、颈痛量表(NPQ)评分、中医证候积分,并评定临床疗效。结果治疗后,2组各项评分均较治疗前降低,P<0.05,且联合组均低于埋线组,P<0.05。联合组治疗总有效率高于埋线组(P<0.05)。结论黄连温胆汤加减联合穴位埋线可以明显改善颈部不适引起的睡眠障碍,且疗效优于单纯穴位埋线治疗。Objective To observe the clinical effect of Huanglian Wendan decoction combined with acupoint catgut embedding in the treatment of cervical insomnia caused by phlegm⁃heat attacking internally type.Methods From January 2021 to December 2022,60 patients with cervical insomnia caused by phlegm⁃heat attacking internally type in Huacao Community Health Service Center of Minhang District were selected.They were randomly divided into combined group and acupoint catgut embedding group,with 30 cases in each group.The acupoint catgut embedding group was given acupoint embedding treatment,and the combined group was given Huanglian Wendan decoction combined with acupoint embedding treatment.After a total of 8 weeks of treatment,Pittsburgh Sleep Quality Index(PSQI)score,Neck Pain Scale(NPQ)score and TCM syndrome score of the two groups were compared,and the clinical efficacy was evaluated.ResultsAfter treatment,all scores in 2 groups were lower than before treatment(P<0.05).The combined group was lower than the acupoint catgut embedding group(P<0.05).The total effective rate of combined group was higher than that of acupoint catgut embedding group(P<0.05).Conclusion Huanglian Wendan decoction combined with acupoint catgut embedding can significantly improve sleep disorders caused by neck discomfort,and its therapeutic effect is superior to simple acupoint catgut embedding.
