摘　　要：目的 探究中药保留灌肠对直肠内脱垂(IRP)吻合器痔上黏膜环切(PPH)术后患者生活质量及对直肠黏膜TRPV1、TRPV4的影响。方法 选取2021年1月—2021年12月入住福建中医药大学附属第二人民医院肛肠科的80例直肠内脱垂(IRP)患者,均行PPH手术,随机分为观察组和对照组,各40例。术后观察组予中药保留灌肠疗法,对照组予生理盐水灌肠。比较2组术后当天、术后7 d直肠黏膜TRPV1、TRPV4表达水平;2组术后并发症发生率;术前、术后30 d 2组生活质量评分。结果 术后7 d 2组直肠黏膜TRPV1、TRPV4表达值均低于治疗前,且观察组均低于对照组(P<0.05);术后观察组并发症发生率低于对照组(P<0.05);术后30 d 2组SF-36评分均高于治疗前,且观察组高于对照组(P<0.05)。结论 IRP患者在行PPH术后予中药保留灌肠可降低直肠黏膜TRPV1、TRPV4表达水平,减少术后并发症,显著改善IRP患者术后生活质量。Objective To explore the effect of Chinese medicine retention enema on the quality of life and its influence on TRPV1 and TRPV4 expression in the rectal mucosa with intrarectal rectal prolapse(IRP)after PPH.Methods 80 IRP patients who underwent PPH surgery in The Second People's Hospital Affiliated to Fujian University of Traditional Chinese Medicine from January 2021 to December 2021 were randomly divided into observation group of 40 cases and control group of 40 cases.The observation group received Chinese herbal enema therapy after surgery,while the control group received saline enema therapy.The TRPV1 and TRPV4 expression levels in the rectal mucosa were compared between the two groups before and 7 days after surgery.The incidence of postoperative complications and quality of life scores were also compared between the two groups before and 30 days after surgery.Results The TRPV1 and TRPV4 expression levels in the rectal mucosa were lower in both groups 7 days after surgery compared to before surgery,and the levels in the observation group were significantly lower than those in the control group(P<0.05).The incidence of postoperative complications was lower in the observation group than in the control group(P<0.05).The SF-36 scores in both groups were higher than before surgery 30 days after surgery,and the scores in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Chinese medicine enema therapy can reduce TRPV1 and TRPV4 expression levels in the rectal mucosa,reduce postoperative complications,and significantly improve the quality of life of IRP patients after PPH surgery.
