摘 要:目的探究象皮生肌膏对大鼠肛瘘切除术后创面模型的治疗作用及其与磷脂酰肌醇3-激酶/蛋白激酶B(Phosphoinosi-tide 3-kinase/proteinkinase B,PI3 K/Akt)信号通路的相关性。方法将36只SPF级健康成年雄性SD大鼠按照随机数字表法分为假手术组、模型组、象皮生肌膏组、湿润烧伤膏组,每组各9只。除假手术组外均采用0号钢丝(Aciflex)制成人造肛瘘,随后进行肛瘘切除术,创面不缝合,保持开放,使之形成一个“急性、开放、渗血”的类似临床肛瘘术后创面,并予以相应药物肛周换药10 d,模型组予以灭菌注射用水换药。利用常规面积检测法比较模型组、象皮生肌膏组、湿润烧伤膏组各组大鼠肛瘘术后创面的创面愈合率;苏木精-伊红(He-matoxylin-eosin staining,HE)染色观察各组大鼠肛周组织病理情况;蛋白免疫印迹法(Western blot,WB)比较各组大鼠肛周肉芽组织中白介素17(Interleukin 17,IL-17)、磷脂酰肌醇3-激酶(Phosphoinositide 3-kinase,PI3 K)、蛋白激酶B(Proteinkinase B,Akt)、磷酸化PI3 K(Phospho-PI3 K,p-PI3 K)、磷酸化Akt(Phospho-AKT,p-AKT)蛋白表达水平;定量实时聚合酶链锁反应(Quantitative real time polymerase chain reaction,Q-PCR)检测各组大鼠肛周肉芽组织中IL-17、PI3 K、Akt mRNA的表达及象皮生肌膏的干预作用。结果与假手术组比较,模型组大鼠创面组织中IL-17、p-PI3 K、p-Akt蛋白表达明显升高,差异有统计学意义(P<0.05);与模型组比较,象皮生肌膏组、湿润烧伤膏组大鼠创面组织中IL-17、p-PI3 K、p-Akt的蛋白表达明显降低,差异有统计学意义(P<0.05),且象皮生肌膏组大鼠创面组织中IL-17、p-PI3 K、p-Akt的蛋白表达低于湿润烧伤膏组,差异有统计学意义(P<0.05)。与假手术组比较,模型组大鼠创面组织中IL-17、PI3 K、Akt的mRNA表达明显升高,差异有统计学意义(P<0.05);与模型组比较,象皮生肌膏组、湿润烧伤膏组大鼠创面组织中IL-17、PI3 K、Akt�Objective To explore the therapeutic effect of Xiangpi Shengji Ointment on the wounds of rats after anal fistula resection and its correlation with phosphoinositide 3-kinase/protein kinase B(PI3 K/Akt)signaling pathway.Method A total of 36 SPF-grade healthy adult male SD rats were randomly divided into a sham-operated group,model group,Xiangpi Shengji ointment group and moisture exposed burn ointment(MEBO)group,with 9 rats in each group.Except for the sham-operated group,all groups underwent artificial anal fistula creation using 0-gauge steel wire(Aciflex),followed by anal fistula resection,with the wound left open and un-sutured to create“acute,open,and bleeding”postoperative anal fistula-like wound.These wounds were treated with corresponding medications for 10 days,while the model group received sterile injectable water.The wound healing rates of rats in model group,Xiangpi Shengji ointment group and MEBO group after anal fistula operation was compared by conventional area measurement method;hematoxylin-eosin staining(HE)was used to observe the pathology of perianal tissues of rats in each group;western blot(WB)was used to compare the protein expression levels of interleukin-17(IL-17),phosphatidylinositol 3-kinase(PI3 K),protein kinase B(Akt),phosphorylated PI3 K(p-PI3 K),and phosphorylated Akt(p-AKT)in perianal granulation tissues of rats in each group;quantitative real-time polymerase chain reaction(Q-PCR)was used to detect the mRNA expression of IL-17,PI3 K,Akt in perianal granulation tissue of rats in each group and the intervention effect of Xiangpo Shengji Ointment.Results Compared with the sham-operated group,the model group exhibited significantly increased protein expression of IL-17,p-PI3 K,and p-Akt in the wound tissue(P<0.05);compared with the model group,the Xiangpo Shengji Ointment group and MEBO group showed significantly reduced protein expression of IL-17,p-PI3 K,and p-Akt(P<0.05),with the Xiangpo Shengji Ointment group demonstrating lower protein expression of IL-17,p-PI3 K,p-Akt than th
