梅花针刺疗法结合面肌功能训练改善周围性面神经麻痹患者的面神经功能、社会功能的研究

摘　　要：目的探讨梅花针刺疗法结合面肌功能训练对周围性面神经麻痹患者的面神经功能、社会功能的影响。方法选取2021年6月—2022年12月期间清远市人民医院康复医学科及儿科门急诊部收治的90例周围性面神经麻痹患者为研究对象,根据随机数字表法分为对照组和观察组,每组各45例。对照组予常规治疗,观察组在对照组基础上予以梅花针刺疗法结合面肌功能训练,每7 d为1个疗程。连续治疗两个疗程后,观察比较两组患者治疗前后改良Portman评分、面部对称性评分、House-Brackmann(H-B)面神经功能评分、生活质量EORTCQLQ-C30V 3.0量表评分。结果治疗后两组患者两侧6种面部运动改良Portman评分均较治疗前升高,差异有统计学意义(P<0.05);且观察组高于对照组,差异有统计学意义(P<0.05)。治疗后两组患者静态下及随肌运动下面部对称性评分均较治疗前降低,差异有统计学意义(P<0.05);且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者面神经功能恢复情况H-B评分均较治疗前降低,差异有统计学意义(P<0.05);且观察组明显低于对照组,差异有统计学意义(P<0.05)。治疗后两组患者生活质量EORTCQLQ-C30V 3.0评分较治疗前明显升高,差异有统计学意义(P<0.05);且观察组明显高于对照组,差异有统计学意义(P<0.05)。结论梅花针刺联合面肌功能训练能有效促进周围性面神经麻痹患者面神经功能恢复,增强面肌运动功能,改善面部对称性,有助于改善患者社会功能,提高其生活质量。Objective To explore the effect of plum blossom acupuncture combined with facial muscle function training on the facial nerve function and social function of patients with peripheral facial nerve paralysis.Method A total of 90 patients with peripheral facial paralysis admitted to the Rehabilitation Medicine Department and Pediatric Department of Qingyuan People′s Hospital between June 2021 and December 2022 were selected as the study subjects.Using a random number table method,the patients were divided into a control group and an observation group with 45 patients in each group.The control group received routine treatment,while the observation group received plum blossom acupuncture therapy combined with facial muscle function training on top of the control group′s treatment.Each treatment course lasted for 7 days.After two consecutive courses of treatment,the improvement in Portman score,facial symmetry score,House-Brackmann(H-B)facial nerve function score,and quality of life(evaluated using the EORTC QLQ-C30 V3.0 scale)were compared between the two groups before and after treatment.Result After treatment,the Portman scores for the improvement of six facial movements on both sides in both groups significantly increased compared to before treatment(P<0.05).Additionally,the observation group showed significantly higher scores than the control group(P<0.05).The facial symmetry scores under static and dynamic muscle movement conditions in both groups decreased after treatment compared to before treatment,and the differences were statistically significant(P<0.05).The observation group had significantly lower scores than the control group(P<0.05).After treatment,the H-B scores for facial nerve function recovery in both groups remarkably decreased compared to before treatment(P<0.05).Moreover,the observation group had significantly lower scores than the control group(P<0.05).The EORTCQLQ-C30V3.0 scores for both groups increased significantly after treatment compared to before treatment(P<0.05).The score of the obse
