从肺论治尿毒症皮肤瘙痒症的思路探讨

摘　　要：尿毒症皮肤瘙痒症(Uremic pruritus, UP)为维持性血液透析(Maintenance hemodialysis, MHD)患者的常见并发症,严重影响其生活质量和治疗结局,是病死率的独立预测因素。目前,西医的治疗尚没有统一的方案,疗效有限。中医治疗手段的多样化为治疗提供新的思路。而从肺论治UP的治则治法包括补肺法、润肺法、清肺法、宣肺法等,根据辨证兼以补益脾肾、活血化瘀、清利湿浊及祛风等治法可明显减轻临床症状,提高患者生活质量。Uremic pruritus(UP)is a common complication in maintenance hemodialysis(MHD)patients,which seriously affects their quality of life and treatment outcomes.It is an independent predictor of mortality.At present,there is no unified treatment plan for Western medicine,and the therapeutic effect is limited.The diversification of traditional Chinese medicine treatment methods provides new ideas for treatment.The treatment of UP from the perspective of the lung includes tonifying,moistening,clearing,and promoting the lungs.Based on syndrome differentiation,the treatment of tonifying the spleen and kidney,promoting blood circulation and resolving stasis,clearing dampness and turbidity,and dispelling wind can significantly reduce clinical symptoms and improve patients′quality of life.
