郑进治疗咳嗽组方用药规律研究

摘　　要：目的:探究郑进教授治疗咳嗽的临床用药组方规律。方法:运用回顾性研究方法,收集郑进教授诊治的139例咳嗽患者,共212张处方,将方药信息经模板数据规范化处理后录入中医传承计算平台(V3.0),运用“数据分析”内的“统计分析”和“方剂分析”探讨郑进教授治疗咳嗽的处方用药和配伍规律。结果:212张处方共包含207味药物,出现频次在30次以上的药物共有34味,四气以温、寒、平性为主,五味以苦、辛、甘味为主,归经多以肺、胃、脾、心、肝经为主,功效以化痰止咳平喘、补虚、解表、清热、理气为主。基于关联规则算法分析得出核心药物组合多为止嗽散化裁而成。郑进教授将化裁后的药物组方命名为“麻杏止嗽散”;运用聚类分析结合郑进教授临床经验得出核心新处方5首。结论:郑进教授治疗咳嗽早期主要以“气郁”“痰郁”,中后期以久咳“气虚”“津伤”等为关键病机,以“畅”为通为用,辨证施治,以畅达气机、复肺宣肃、化痰止咳、温补肺气为基本治法,注重脾胃气机枢纽和肝脏调畅气机等功能,同时结合滇南民族用药提高临床疗效。Objective:To explore the clinical prescription rules of Professor Zheng Jin in the treatment of cough.Method:A retrospective study was conducted to collect 212 prescriptions from 139 patients diagnosed and treated with cough by Professor Zheng Jin.The above prescription information was standardized using template data and entered into the Traditional Chinese Medicine Inheritance Calculation Platform(V3.0).The"statistical analysis"and"prescription analysis"in"data analysis"were used to explore the prescription medication and compatibility rules of Professor Zheng Jin in treating cough.Results:A total of 207 medicines were included in 212 prescriptions,with a total of 34 medicines appearing more than 30 times.The Four Qi of them are mainly warm,cold and mild,while the Five Taste of them are mainly bitter,spicy,and sweet.The meridians tropism are mainly focused on the Lung,Stomach,Spleen,Heart and Liver meridians,with the main effects of resolving Phlegm,stopping cough,relieving asthma,tonifying deficiency,relieving exterior evil factor,clearing Heat and regulating Qi.Based on the analysis of association rule algorithm,it was found that the core drug combination was made by modifying the formula of Zhisou Powder.Professor Zheng Jin named the modified drug combination as"Maxing Zhisou Powder;Cluster analysis was made and by combing with Professor Zheng Jin′s clinical experience,5 core new prescriptions were identified.Conclusion:Professor Zheng Jin′s early treatment of cough mainly focuses on"Qi stagnation"and"Phlegm stagnation",while in the middle and later stages,the key pathogenesis of chronic cough is"Qi deficiency"and"Fluit injury".The treatment policy of"dredging"is used based on syndrome differentiation.The basic treatment methods include unblocking the Qi mechanism,restoring the Lung Qi dispersing,resolving Phlegm and stopping cough,warming and tonifying the Lung Qi,emphasizing the functions of the Spleen and Stomach Qi hub and regulating the Liver Qi mechanism.At the same time,combined with medication fo
